FESS联合MMT对中-重度CRSsNP的疗效及对鼻腔黏液纤毛清除能力的影响

摘　　要：目的:探讨功能性内镜鼻旁窦手术(FESS)联合加强药物治疗(MMT)对中-重度慢性鼻-鼻旁窦炎不伴鼻息肉(CRSsNP)的疗效及对鼻腔黏液纤毛清除能力的影响。方法:选取我院治疗的CRSsNP患者120例为研究对象,按照随机数字表法分为研究组与对照组各60例,对照组予MMT治疗,研究组在对照组基础上予FESS治疗,比较两组患者的疗效和鼻腔黏液纤毛清除能力。结果:研究组患者的总有效率显著高于对照组(P<0.05)。治疗后4及8周,两组患者的糖精清除时间呈降低趋势,研究组显著低于对照组(P<0.05),而两组患者的黏液纤毛清除速度及腔黏液纤毛清除率呈上升趋势,研究组显著高于对照组(P<0.05)。结论:FESS联合MMT对中-重度CRSsNP具有较好的临床疗效,能够显著提高患者鼻腔黏液纤毛清除能力,值得临床推广。Objective： To explore the influence of functional endoscopic sinus surgery （FESS） combined with maximal medical therapy （MMT） on clinical effect and mucociliary clearance ability in patients with chronic rhinosinusitis without nasal polyps （CRSsNP）. Methods： One hundred and twenty patients with CRSsNP were divided into study group and control group with 60 cases in each group. The control group was treated with MMT, while the study group was treated with FESS ＋ MMT. The clinical effect and mucociliary clearance ability of the two groups were evaluated. Results： The total effective rate in the study group was significantly higher than that in control group（ P 〈 0. 05）. After treatment for 4 and 8 weeks, it showed that the speed and the rate of mucociliary clearance in the study group were significantly higher than those in the control group （ P 〈 0. 05 ）, while the saccharin clearance time of study group was significantly shorter than that of the control group （P 〈 0.05 ）. Conclusion： The application of FESS combined with MMT therapy shows good clinical outcomes and helps to improve the mucociliary clearance ability, which is worthy of clinical application.
