D-二聚体检测性能评价及其对急性脑梗死患者病情评估的价值

摘  要:目的对免疫比浊法检测D-二聚体的检测性能进行评价,并探讨其对急性脑梗死患者病情评估的价值。方法对263例急性脑梗死患者按美国卫生研究院脑卒中量表评分进行分组,比较不同程度病情急性脑梗死患者组血清D-二聚体含量及与健康对照组的差异。结果 D-二聚体的批内精密度N值和P值的CV分别为4.84%、4.04%,批间精密度N值和P值的CV分别为6.0%、4.74%;线性回归方程Y=1.044 1 X-0.217,R2=0.991;携带污染率0.26%,该检测系统性能良好。脑梗死组患者的血清D-二聚体含量显著高于对照组(P<0.05),其中重度、中度脑梗死组患者血清D-二聚体的含量显著高于轻度脑梗死组(P<0.05)。结论免疫比浊法D-二聚体检测性能符合要求,能够为临床提供可靠的结果;D-二聚体对急性脑梗死患者临床诊断有重要意义。Objective To evaluate the performance of an immunoturbidimetric assay for D - dimer and its clinical application in acute stroke. Methods 263 stroke patients were recruited and divided into two groups according NIH Stroke Scale Booklet. We evalu- ated the level of D - dimer between the two groups. Results The Performance evaluation had the following characteristics :inter - assay imprecision : Value of D - dimer within batch precision N and P value of CV was 4.84% ,4.04% and inter batch precision n value and P value of CV to 6.0% ,4.74% ;linear regression equation Y = 1. 0441X - 0.217, R = 0.991 ; carryover contamination rate of 0.26%. The instrument has good performance and can be used for clinical detection. The level of D - dimer was significantly higher than the control group ( P 〈 0.05 ). It was the highest in the patients with severe acute stoke ( P 〈 0.05 ). Condtmion Performance of an immunoturbi- dimetric assay for D - dimmer can provide reliale resultsl Detection the D - dimer of acute stoke cerebral infarction patients has great sig- nificance to clinical diagnosis and analysis.
