摘  要:目的观察静脉注射氢盐水对大鼠机械通气相关性肺损伤模型的影响。方法健康SD大鼠36只,体质量250~360g,随机分为对照组(C组)、机械通气组(MV组)和氢盐水干预组(H组)。C组不进行机械通气,MV组和H组进行机械通气(VT40mL/kg、RR40次/min)4h。H组于机械通气时经股静脉注射氢盐水(1.0mL/100g),其余两组同时间点注射等量的生理盐水。所有动物于机械通气4h后放血处死大鼠,取肺组织,计算肺湿,干质量比(W/D),光镜下观察肺组织病理学改变,检测肺组织髓过氧化物酶(MPO)活性、丙二醛(MDA)和超氧化物歧化酶(SOD)含量;收集支气管肺泡灌洗液(BALF),检测总蛋白含量、TNF-α和IL—1β浓度以及白细胞计数。结果与C组比较,MV组和H组大鼠肺W/D、MPO活性、BALF中总蛋白含量、TNF—α和IL-1β浓度以及WBC均显著升高(P〈0.05),且H组升高低于MV组(P〈0.05)。与C组比较,MV组肺组织MDA含量显著升高(P〈0.01),SOD含量显著降低(P〈0.01);和MV组比较,H组肺组织MDA含量显著降低(P〈0.01),SOD含量显著升高(P〈0.01)。结论静脉注射氢盐水可能通过抗炎和抗氧化机制显著减轻大鼠机械通气相关性肺损伤。Objective To investigate the effect of intravenous injection of hydrogen-rich saline on mechanical ventilation induced lung injury in rats. Methods Thirty-six healthy SD rats weighting 250-360 g were randomly divided into 3 groups (n = 12 each): control group (group C), large tidal volume mechanical ventilation group (group MV) and treatment of hydrogen-rich saline group (group H). Group C received no mechanical ventilation. In group MV and H the animals were mechanically ventilation for 4 h (VT 40 mL/kg, RR 40 bpm). In group H hydrogen-rich saline ( 1.0 mL/100 g) were administered iv at the mechanical ventilation. Equal amoumt of normal saline was given to rats in other two groups at the same time. The animals were mechanically ventilated for 4 h and then sacrificed. The sample of pulmonary tissue and lung lavage fluid were collected after experiments. Lung pathological changes were examined with optical microscopy. The expression of cytokines TNF-α and IL-1β in BALF were measured by ELISA assay. Total protein contents, wet/dry ratios (W/D), myeloperoxidase (MPO) activity, MDA concentration, SOD activity and white blood cells counts of the lung tissue or lavage fluid were measured with corresponding methods. Results As compared with group C, total protein contents, W/D, TNF-α, IL-1β, myeloperoxidase (MPO) activity, MDA concentration and neutrophil counts were significantly higher in group MV. However, SOD activity in lung tissue was significantly decreased in group MV than group C. Treatment with hydrogen-rich saline attenuated ventilator-induced lung injury, total protein contents, W/D, TNF-α, IL-1β, MPO activity, MDA concentration and neutrophil counts caused by high volume ventilation. But SOD activity in lung tissue was significantly increased in group H. Con- clusion Intravenous injection of hydrogen-rich saline can ameliorate mechanical ventilation induced lung injury in rats to some extent, which is associated with means of anti-inflammatory and antioxidant effe
