摘 要:目的:比较1%丙泊酚与2%丙泊酚用于腹腔镜胆囊切除术患者的药效学及药物用量。方法:选择拟行腹腔镜胆囊切除术患者80例,随机分成2组,每组40人;Ⅰ组输注1%丙泊酚,Ⅱ组输注2%丙泊酚,诱导浓度为3μg/m L,用听觉诱发电位监护仪控制镇静水平,术中维持AAI指数在20左右(15~25)。观察患者意识消失时间、AAI下降到30的时间、MAP平均下降百分比、HR平均下降百分比、丙泊酚诱导用量、丙泊酚第1小时用量及AAI恢复到60的时间。结果:AAI下降到30的时间Ⅰ组为(194.75±84.81)s,Ⅱ组为(236.75±92.49)s,Ⅰ组短于Ⅱ组(t=-2.04,P=0.04),丙泊酚诱导用量I组为(9.97±2.64)m L,Ⅱ组的2倍为(12.20±2.60)m L,Ⅰ组小于Ⅱ组的2倍(t=-3.45,P=0.00)、丙泊酚第1小时用量I组为(37.83±9.94)m L,Ⅱ组的2倍为(44.21±13.99)m L,Ⅰ组小于Ⅱ组的2倍(t=-2.30,P=0.02),丙泊酚使用总量I组为(49.18±22.62)m L,Ⅱ组的2倍为(62.07±24.88)m L,Ⅰ组小于Ⅱ组的2倍(t=-2.97,P=0.00)。结论:1%丙泊酚用量(毫升数)小于2%丙泊酚的2倍,说明1%丙泊酚的药效可能强于2%丙泊酚。Objective:To compare the pharmacodynamics between 1% propofol and 2% propofol during anesthesia of patients with laparoscopic cholecystectomy. Methods:Totally 80 patients with laparoscopic cholecystectomy were randomly divided into two groups.Group Ⅰ was given target controlled infusion of 1% propofol(3μg/m L) for anesthesia induction and group Ⅱ was given 2%propofol(3 μg/m L),auditory evoked potentials index(AAI)value was maintained below 30 during the whole procedures.The time of consciousness extinction,the time of AAI value decreasing to 30,average arterial pressure(MAP) mean decreasing percent,heart rate(HR) mean decreasing percent,dosage of propofol,dosage of propofol at 1 hour and the time of AAI value recuperating to 60 were recorded and assessed. Results:The time of AAI value decreasing to 30 was(194.75±84.81)s and(236.75±92.49)s in Ⅰ group and Ⅱgroup respectively(t=-2.04,P=0.04).The dosage of propofol was higher in Ⅱgroup[(12.20±2.60)m L] than in Ⅰ group[(9.97±2.64)m L](t=-3.45,P=0.00). The dosage of propofol at 1 hour was higher inⅡ group[(44.21±13.99)m L] than in Ⅰ group[(37.83±9.94)m L](t=-2.30,P=0.02). The total dosage of propofol was higher inⅡ group[(62.07±24.88)m L] than in Ⅰ group [(49.18±22.62)m L](t=-2.97,P=0.00). Conclusion:The potency of 1% propofol is stronger than 2% propofol.
