乌司他丁联合血必净对海水浸泡状态下兔腹部枪弹伤救治作用研究

摘　　要：目的探讨乌司他丁联合血必净对海水浸泡状态下兔腹部枪弹伤的救治作用。方法 30只腹部枪弹伤在海水浸泡状态下的新西兰兔,随机均分为对照组(只施行常规救治)、实验组(常规救治同时加用乌司他丁联合血必净),分别于伤后1、3、5 d,观察两组肛温(rectal temperature,RT)、心率(heart rate,HR)、平均动脉压(mean arterial pressure,MAP)、白细胞(white blood cell,WBC)、C反应蛋白(C-reaction protein,CRP)以及5 d死亡率等指标。结果实验组治疗后在各观察时间点RT、HR均较对照组明显下降,MAP较对照组有明显升高,差异均具有统计学意义(P<0.05);实验组WBC、CRP于伤后各时间点均明显低于对照组,差异有统计学意义(P<0.05);伤后5d实验组和对照组死亡率分别为20.0%和60.0%,差异有统计学意义(P<0.05)。结论乌司他丁联合血必净可以稳定海水浸泡腹部枪弹伤兔的生命体征,同时减轻炎症反应程度、降低死亡率。Objective To investigate the treatment effect on ulinastatin and Xuebijing in abdominal gunshot wound of rabbit model in seawater immersion condition.Methods Thirty New Zealand gunshot wounded rabbits in seawater immersion condition were randomly divided into control group（regular treatment only）、experimental group（regular treatment combined with ulinastatin and Xuebijing at the same time）.The indexs of two groups were respectively mearsured after 1-,3-,and 5-day injury,including rectal temperature（RT）,heart rate（HR）,mean arterial pressure（MAP）,white blood cell（WBC）,c-reactive protein（CRP）,5-day mortality and so on.Results RT and HR after treatment were significantly lower at each observation time point and MAP was significantly increased in experimental group（P〈0.05）.WBC and CRP after injure in experimental group were significantly lower than these in control group at each observation time point（P〈0.05）.5-day mortality after injure in experimental group and control group were respectively 20.0 % and 60.0 %,the difference was statistically significant（P〈0.05）.Conclusion Ulinastatin combining with Xuebijing can make vital signs of abdominal gunshot wounded rabbits stable with seawater immersion condition,meanwhile,reduce inflammatory and mortality.
