摘 要:目的:探讨磁共振弥散加权成像(DWI)结合体积测量在婴幼儿梗阻性肾积水肾功能评价中的应用价值。方法:对20例健康儿童及80例单侧肾积水患儿行MR检查,测量肾脏的表观弥散系数(ADC)及肾脏体积,记录患儿的血清肌酐值并计算内生肌酐清除率(CCr),并对各参数进行相关性分析。结果:病例组患儿积水侧肾脏ADC值均低于对侧肾脏组及正常对照组(P<0.05),积水肾的ADC值与CCr具有正相关性(r=0.555,P<0.05),经体积校正后积水肾ADC值体积乘积与CCr间正相关性明显提高(r=0.868,P<0.05)。结论:ADC值与肾脏体积乘积值对于评估婴幼儿梗阻性肾积水肾功能有一定临床价值。Purpose: To investigate the value of MR diffusion weighted imaging combined with the measurement of renal volume in the evaluation of the renal function in infants with obstructive hydronephrosis. Methods: Twenty healthy volunteers and 80 patients with unilateral obstructive hydronephrosis underwent conventional magnetic resonance sequences and DWI. The ADC values of the renal parenchyma and the renal volume were measured on DWI and conventional MR sequences. The serum creatine level was recorded and the creatine clearance rate was calculated. Correlation between these parameters was analyzed. Results: ADC values of the renal parenchyma in patients with renal hydronephrosis was significantly lower than ADC values of the contralateral normal kidney and the normal control group (P〈0.05); There was a positive correlation between ADC value of the renal parenchyma in hydronephrotic kidney and CCr in patients with renal hydronephrosis (r=0.555, P〈0.05); The positive correlation was increased significantly between volume corrected ADC value and CCr in patients with renal hydronephrosis (r=0.868, P〈0.05). Conclusion: The product of ADC value and volume of the hydronephrotic kidney has a certain clinical value for the evaluation of the function of hydronephrotic kidney in infants with congenital hydronephrosis.
