西格列汀对2型糖尿病患者传统和非传统心血管危险因素的影响

摘　　要：目的观察西格列汀对磺脲类或胰岛素治疗的2型糖尿病患者血糖、血压、血脂、体重等传统的心血管危险因素以及血清炎症因子和氧化应激指标的非传统心血管危险因素的影响。方法选取单用格列齐特、格列美脲、胰岛素或联合二甲双胍治疗血糖控制欠佳的2型糖尿病患者59例。随机分成西格列汀组及安慰剂对照组,两组分别在原有治疗方案的基础上加用西格列汀或相应的安慰剂,共治疗24周。结果西格列汀治疗24周后,糖化血红蛋白(Hb A1C)、空腹血糖(FBG)、餐后2 h血糖(2 h PBG)、体重、收缩压(SBP)、C反应蛋白(CRP)、白细胞介素6(IL-6)、肿瘤坏死因子α(TNF-α)、氧化应激指标(8-iso-PGF2α)较治疗前均有下降,差异具有统计学意义(P<0.05)。结论西格列汀对磺脲类或胰岛素治疗的2型糖尿病患者的血糖、血压、部分脂代谢指标等传统的心血管危险因素都有改善作用,并能抑制炎症反应及改善氧化应激状态。Objective To investigate the effects of sitagliptin on traditional cardiovascular risk factors and non- traditional cardiovascular risk factors in type 2 diabetes patients treated with sulfonylurea or insulin. Methods A number of 59 patients of type 2 diabetes with blood glucose poorly controlled undertook gliclazide, glimepiride, insulin alone or in combination with metformin therapy were divided into test group and control group. The test group provided sitagliptin 100mg qd with original therapy while the control group provided the placebo. Results After 24 weeks, HbA1C, FBG, 2 h PBG, body weight, SBP, TG, CRP, IL-6, TNF-α and 8-iso-PGF2α were significantly reduced （P 〈 0.05）. Conclusions Sitagliptin has improvement effect on traditional cardiovascular risk factors （such as blood glucose, blood pressure, triglyeeride and body weight） and exerted ant i-atherosclerotic effect by restraining inflammation and ameliorating oxidative stress in type 2 diabetes patients treated with sulfonylurea or insulin.
