基于系统聚类分析总结吴良村辨治结直肠癌经验

摘　　要：目的:总结吴良村辨治结直肠癌的用药规律。方法:文章纳入2011年7月1日-2014年6月30日吴良村教授门诊304例结直肠癌患者,608诊次,将患者临床信息转化为方剂基础数据库,对药物累计频数、药物性味采用描述性分析,对频数最高的50种药物进行系统聚类分析。结果:304例患者608诊次共用药224种,累计用药频次11 506,其中用药频次最多者为茯苓,其后依次为鸡内金、白术、山药、藤梨根、白花蛇舌草、石斛、甘草。药性使用频次最多的为"平",药味使用频次最多的为"甘"。聚类分析将药物分为21类,涉及方剂四君子汤,药对知母和生地黄、炒稻芽和麦芽、太子参和炒薏苡仁、杜仲和天麻等。结论:吴良村辨治结直肠癌以健脾益气为主,随证辅以熄风通络、活血化瘀、解毒抗癌、消食和中之法,临证用药善于甘、苦、辛、平配伍。Objective：To summarize WU Liang-cun＇s experience in differentiating and treating rectal carcinoma.Methods：Three hundreds and four outpatient patients with rectal carcinoma with 608 floe rates from July 1,2011 to June 30,2014 were included.The clinical information of patients was transformed to the prescription base database.The cumulative frequency and nature and flavour of drugs were analyzed by using descriptive analysis,and the 50 drugs with the highest frequency were clustered systematically.Results：There were 224 drugs were used in 304 patients with 608 flow rates.The accumulation frequency was 11506,in which the drug with the highest frequency was indian bread,followed by inner membrane of chicken gizzard,largehead atractylodes rhizome,common yam rhizome,Chinese actinidia root,hedyotis,dendrobium,and liquorice root.The drug property with the highest frequency was calm,and the drug flavor with the highest frequency was sweet.The drugs were divided into 21 categories by clustering analysis involving prescription as Sijunzi Decoction,herb pairs as common anemarrhena rhizome and unprocessed rehmannia root,millet sprout and germinated barley,heterophylly falsestarwort root and rice kernels,eucommia bark and tall gastrodia tuber,etc.Conclusion：WU Liang-cun＇s experience in treating rectal carcinoma gives priority to invigorating spleen for benefiting qi,and supplementes by calming endogenous wind and dredging collaterals,promoting blood circulation for removing blood stasis,removing toxic substance and anti-cancer,and resolving food stagnation for regulating stomach.He is good at using medicine compatibilities as sweet,bitter,pungent and calm.
