基于聚类分析的支气管哮喘慢性持续期证候特征研究

摘　　要：目的:研究支气管哮喘(BA)慢性持续期的证候特征。方法:采用SPSS 20.0软件,对736例通过多中心、前瞻性、开放式、有监督无监督相结合的观察性研究方法采集BA慢性持续期患者的证候与症状、体征、舌苔、脉象进行聚类分析。结果:症状及体征可聚为5类:第一类:面色胱白,大便溏,精神倦怠,纳呆,眠差,乏力,少气懒言,畏风寒;第二类:咳嗽,胸膈满闷,咯痰,哮鸣音,喘息;第三类:脘腹胀满,痰黏难咳,胸疼,烦躁,口渴,焦虑,抑郁;第四类:耳鸣,腰膝酸软,眩晕,鼻痒,咽痒,喷嚏,自汗,易外感,声音低怯,小便频数;第五类:呼吸困难,喘促气短。舌苔、脉象可聚为五类:第一类:黄苔,数脉;第二类:腻苔,滑脉,滑苔;第三类:舌暗,有瘀斑,结脉,虚脉,弦脉,弱脉,濡脉,实脉,涩脉;第四类:细脉;第五类:舌淡胖,边有齿痕,沉脉,厚苔。结论:慢性持续期主要证候为肺脾两虚、肺肾亏虚、痰热蕴肺、痰湿壅肺,涉及多脏腑的功能失调,尤以肺脾为核心,痰饮、瘀血等病理产物共存,呈现本虚标实、寒热错杂的证候特征。Objective：To explore syndrome characteristics of bronchial asthma（BA） in chronic persistent period.Methods：Seven hudred and thirty-six cases of bronchial asthma patients in chronic persistent period were collected for cluster analysis by SPSS 20.0.Observational research with polycentric,prospective,opened and combined with supervised and unsupervised characteristics was launched to collect information of syndrome,symptoms,signs,tongue coating and pulse of patients.Results：Symptoms and signs were clustered for five categories.The first category was pallid complexion,loose stool,mental burnout,anorexia,weakness,lack of qi and no desire to speak and fear of cold;the second category was coughing,fullness and oppression in the chest and diaphragm,expectoration,wheezing rale,gasping;the third category was abdominal fullness and distention,coughing up phlegm difficultly,chest pain,irritability,thirsty,anxiety,depression;the fourth category was tinnitus,soreness and weakness of waist and knees,vertigo,nose itching,throat itching,sneezing,spontaneous sweating,easy to catch a cold,low timid voice,frequent urination;the fifth category was breathing difficultly,shortness of breath.Fur and pulse were clustered for five categories.The first category was yellow fur and rapid pulse;the second category was greasy fur,slippery pulse and slippery fur;the third category was dark tongue with ecchymosis,irregularly intermittent pulse,feeble pulse,stringy pulse,weak pulse,soft pulse,excess pulse,hesitant pulse;the fourth category was thready pulse;the fifth category was plump pale tongue with white fur,with indentation in margin of the tongue,deep pulse,thick fur.Conclusion：The main syndromes of BA in chronic persistent period are deficiency of both lung and spleen,deficiency of lung and kidney,phlegm-heat accumulated in lung,phlegm-dampness obstructed in lung.BA in chronic persistent period involves a lot of dysfunction of the zang-fu organs,especially in lung and spleen as the core,coexistence of pathological product,such
