摘  要:目的了解住院患者腰椎骨密度(T—score)与激素的相关性。方法选取2013年3月~2016年3月在解放军181医院老年病科住院的患者290例,年龄38~97岁,平均年龄(77.5±13.7)岁。对腰椎骨密度与不同性别患者及患者各年龄组多种激素进行偏相关分析,并对骨质正常组、骨量减少组、骨质疏松(OP)组的激素水平进行比较。结果腰椎T—score与激素偏相关分析:男性与体重、睾酮(TESTO)、C肽(C—P)相关,女性与年龄、身高、体重、游离雌三醇(fEB)、三碘甲状腺原氨酸(T3)相关;年轻老年人组、高龄老年人组腰椎T—score与性别、体重有关,与年龄均不相关;年轻老年人组腰椎T—score与身高相关;所有老年人腰椎T—score均与雌二醇(E2)、C—P相关;高龄老年人组腰椎T—SCOFe均与游离三碘甲状腺素(FT3)有关:年轻老年人组股骨骨密度T—Score与甲状腺素(T4)、甲状旁腺激素(PTH)相关(P〈0.05)。男性激素与女性比较.TESTO、E2、fEB、TB、T4、生长激素(GH)、C-P等差异均有统计学意义(P〈0.05);OP、骨量减少、骨质正常组3组比较,性别、年龄、身高、体重、E2、C-P差异均有统计学意义(P〈0.05)。骨质正常组与骨量减少组比较,性别、身高、体重差异均有统计学意义(P〈0.05),骨量减少组与骨质疏松组比较,只有体重差异有统计学意义(P〈0.05),骨质正常组与骨质疏松组比较,性别、身高、体重、TESTO、E2、FT4、C—P差异均有统计学意义(P〈0.05)。结论腰椎骨密度与老年人的性别、体重相关,与年轻老年人组身高相关。所有老年人组腰椎骨密度与E2、C—P相关,部分老年人腰椎骨密度与FTB、T4、PTH相关;骨质正常与骨质疏松组比较,女性、身材高、体重轻、TESTO、E2、C—P减少均对骨质疏松有影响。Objective To understand the correlation between bone mineral density and hormones in the lumbar spine. Methods The lumbar bone mineral density (T-score) in partial correlation analysis was compared in patients of different genders, and the hormone levels were compared in a healthy group, osteopenia group and osteoporosis (OP) group. Results The T-score of the lumbar spine and hormone levels were partially correlated in the non-elderly group. The lumbar BMD in all age groups was associated with sex and weight,but age was not related. In the younger group,the lumbar T-score and height were related. The T- score of the lumbar spine in all patients was related to estradiol and C peptides. The T-score of the lumbar spine was related to FT3 in the elderly group. The T-score of the lumbar spine in young and old patients was related to T4 and parathyroid hormone (P 〈 0.05). Comparing the hormone levels in men and women,TESTO,E2,fE3,T3,T4,GH, and C-P were statistically significant (P 〈 0.05). In the OP, osteopenia and healthy groups, gender,age, height,weight, estradiol, and C-peptide were statistically significant (P 〈 0.05). There was a significant difference in the gender, height and weight between the healthy, osteopenia and osteoporosis groups. Compared with the osteoporosis group,only the weight of the osteopenia group showed statistical significance. Compared with the osteoporosis group,the levels of testosterone,estradiol,FT4 and C were significantly different in the normal group (P 〈 0.05). Conclusion The bone mineral density of the lumbar spine is related to the gender and weight of elderly patients and is related to the height of younger patients. Bone mineral density of the lumbar spine in all patients is related to estradiol and C-peptide. The bone mineral density of the lumbar spine in some elderly patients is related to FT3,T4 and parathyroid hormone.
