摘　　要：目的利用疑似预防接种异常反应（Adverse Events Following Immunization,AEFI）监测数据探究是否有必要对广东省卡介苗（Bacille Calmette-Guerin,BCG）接种月龄进行推迟调整。方法通过广东省免疫规划信息管理系统和全国AEFI信息管理系统,收集2009～2014年广东省BCG接种数及AEFI个案数据。采用描述性方法对不同接种月龄的不良反应报告发生率等指标进行分析。结果广东省共报告2009～2014年接种BCG后AEFI 511例,报告发生率为44.18/100万剂;其中不良反应456例（89.24%）,报告发生率为39.43/100万剂;不良反应以直径〉1.0cm的淋巴结炎最常见（138例,11.93/100万剂）;全身播散性BCG感染罕见（2例,0.17/100万剂）。在儿童〈1月龄、≥1月龄接种BCG后淋巴结炎报告数分别为118例（11.67/100万剂）、20例（13.72/100万剂）。结论基于AEFI监测数据,没有足够证据表明需要推迟BCG接种月龄,应继续维持新生儿BCG免疫策略。Objective To determine whether to delay the age of Bacille Calmette-Guerin Vaccine（ BCG）administration in Guangdong province based on data from the surveillance system for adverse events following immunization. Methods We collected BCG administration data and reported BCG AEFI data in Guangdong from 2009 to 2014 from the Guangdong Provincial Children Immunization Information System and the National AEFI Information Management System. Descriptive methods were used to analyze the reported incidence rates of vaccine reactions by month of age of BCG immunization. Results A total of511 BCG AEFI cases were reported in Guangdong,2009-2014,for an incidence rate of 44. 18 per million doses; 456（ 89. 24%） cases were vaccine reactions,for an incidence rate of 39. 43 per million doses.Most vaccine reactions were lymphadenitis（ diameter 1. 0cm）（ 138 cases,11. 93 per million doses）.Disseminated BCG infection was rare（ 2 cases,0. 17 per million doses）. Respectively,118（ 11. 67 per million doses） and 20（ 13. 72 per million doses） lymphadenitis cases were reported among children immunized at 1 month of age and in children immunized at ≥1 month of age. Conclusions We found no evidence indicating that the age of BCG immunization should be delayed based on AEFI surveillance.We recommend maintaining the existing BCG immunization strategy.
