摘 要:目的了解本院肿瘤患者痰液分离出革兰阴性杆菌的临床分布以及对常用抗菌药物的耐药情况。方法收集本院两年间痰液标本分离出的革兰阴性杆菌,采用K—B纸片扩散法进行药敏试验,并用WHONET5.6对其耐药性进行分析。结果共分离出革兰阴性杆菌420株,占71.3%;以肺炎克雷伯菌占优势,为39.5%、铜绿假单胞菌为30%、鲍曼不动杆菌为16.4%;其中铜绿假单胞菌对亚胺培南呈现低耐药率,对喹诺酮类药物耐药率也较低,对头孢菌素类、青霉素类及其抑制剂类药物有较高的耐药性;肠杆菌属细菌对碳氢酶烯类药物敏感性较高,对氨苄两林等青霉素类呈现极高耐药性,对其他抗菌药物均呈现不同耐药性。结论痰液的痛原菌仍以革兰阴性杆菌为主,因此应加强对革兰阴性杆菌的耐药性监测,指导临床合理使用抗生素十分重要。Objective To investigate the gram negative bacilli clinical distribution and antimicrobial drug resistance isolated from the sputum of patients with tumor in our hospital. Methods Collected in our hospital in recent two years between isolated from sputum specimens of gram negative bacilli and susceptibility test adopted by K-B method,and by WHONET 5.6 to the drug resistance were analyzed. Results Total gram negative bacilli were isolated from 420 strains, accounting for 71.3%; the dominant species of Klebsiella pneumoniae was 39.5% ,and of Pseudomonas aeruginosa was 30% and Bauman Acinetobacter was 16.4%; The Pseudomonas aeruginosa to imipenem has low rate of resistance, of quinolone resistance rate is low, of cephalosporins, penicillins and its inhibitor drugs had higher resistance; Enterobacteriaceae bacteria of hydrocarbon enzyme alkenes drug sensitivity is higher. To ampicillin two forest penicillin class has very high resistance and to other antibacterial drugs showed different resistance. Conclusion Sputum pathogenic bacteria are gram negative bacilli,so we should strengthen the monitoring of drug resistance of gram negative bacilli against gram, to guide the clinical rational use of antibiotics is very important.
