摘 要:目的探究膝关节炎(OA)患者血浆和关节液网膜素-1(Omentin-1),APJ的内源性配体(Apelin)和生长素(Ghrelin)与关节液中Ⅱ型胶原C端肽(CTX-Ⅱ)表达水平的关系,深入了解膝OA患者的发病机制。方法选取保守治疗膝OA患者50例为研究组,同一时间收治的胫骨平台骨折患者或者新鲜创伤性半月板损伤行关节镜治疗患者15例作为对照组。测量身高、体重,记录血脂水平,采用酶联免疫吸附试验测定血浆和关节液Omentin-1,Apelin和Ghrelin及关节液CTX-Ⅱ浓度,分析患者血浆和关节液Omentin-1,Apelin和Ghrelin与关节液CTX-Ⅱ水平的相关性。结果研究组患者的平均年龄明显高于对照组,差异具有统计学意义(P<0.01),在性别构成比、BMI、腰臀比及血脂功能上无显著差异(P>0.05);与对照组比,研究组血浆Apelin、Ghrelin和关节液Apelin显著降低(P<0.01),关节液Apelin、Ghrelin和血浆Apelin显著升高(P<0.01),关节液CTX-Ⅱ表达水平显著降低(P<0.05)。患者血浆Omentin-1、Ghrelin和关节液Apelin与关节液CTX-Ⅱ水平呈显著正相关(P<0.01),血浆Apelin和关节液Omentin-1、Ghrelin与关节液CTX-Ⅱ水平呈显著负相关(P<0.01);在调整年龄、性别与BMI后,进行偏相关分析发现:患者血浆Omentin-1、Ghrelin和关节液Apelin与关节液CTX-Ⅱ水平呈显著正相关(P<0.01),血浆Apelin和关节液Omentin-1、Ghrelin与关节液CTX-Ⅱ水平呈显著负相关(P<0.01)。结论膝OA患者关节液Omentin-1,Apelin和Ghrelin水平与关节液CTX-Ⅱ表达水平关系密切,可以预测组织结构破坏发生的程度及疾病进展,可作为评价膝OA进展与预后的关键生化因子,同时动态监测其含量变化,要考虑到年龄、性别等的影响。Objective To investigate the relationship between Omentin- 1,Apelin and Ghrelin in the plasma and synovial fluid with CTX- Ⅱ expression level in synovial fluid in patients with knee OA,and insight into the pathogenesis of knee OA. Methods Fifty patients with knee osteoarthritis were selected as study group. 15 cases of patients at the same time hospitalized into our hospital for the fracture of the tibial plateau or fresh traumatic meniscus injury as the control group. After the review of the ethics committee of the hospital clinical trial,the subjects' written consent was obtained,the height and weight were measured. The serum lipid levels were measured. The Omentin- 1,Apelin and CTX- Ⅱconcentrations were measured by enzyme- linked immunosorbent assay. The correlation of Omentin- 1,Apelin and Ghrelin in the plasma and CTX- Ⅱ expression level in synovial fluid was analyzed. Results There were significant differences in age in both groups(P〈0. 01),gender,BMI,waist hip ratio and lipid function were had no difference(P〈0. 05). The plasma Apelin,Ghrelin,and joint fluid Apelin of study group were significantly reduced than those of the control group(P〈0. 01),joint fluid Apelin,Ghrelin and plasma Apelin were significantly increased(P〈0. 01),CTX- Ⅱ expression level in joint fluid between the two groups was different(P〈0. 05). There was significant positive correlation between serum Omentin- 1,Ghrelin and joint fluid Apelin and CTX- Ⅱ level in joint fluid(P〈0. 01),plasma Apelin and joint fluid Omentin- 1,Ghrelin,and joint fluid CTX- Ⅱ level exist significant negative correlation(P〈0. 01). After adjustment for age,sex and BMI,partial correlation analysis found that patients with serum Omentin- 1,Ghrelin,and joint fluid Apelin and joint fluid CTX- Ⅱ level had significant positive correlation(P〈0. 01),plasma Apelin and joint fluid,Ghrelin and joint fluid Omentin- 1,CTX- Ⅱ level exist significant negative correlation(P〈0. 01).
