不同时间段按摩配合穴位注射预防混合痔术后尿潴留的临床观察

摘　　要：目的:比较不同时间段内按摩配合穴位注射治疗混合痔术后尿潴留的疗效差异。方法:将120例混合痔术后患者随机分成对照组、术后4 h治疗组及术后8 h治疗组,每组40例,三组均采取术后常规护理,两个治疗组在术后常规护理的基础上加上手法按摩及足三里穴位局部注射新斯的明。结果:术后4 h治疗组有效率(92.5%)明显高于术后8 h治疗组(77.5%)及对照组(55%)(P<0.05);术后4 h治疗组首次排尿时间(33.6±5.8 min)及平均排尿时间(57.4±2.2 min)明显低于术后8 h治疗组(39.5±4.3 min)、(68.9±1.7 min)及对照组(63.3±7.9 min)、(87.4±5.6 min)(P<0.05);术后4h治疗组首次排尿量(363.6±158.7 m L)及2 h内排尿总量(447.4±198.2 m L)明显高于术后8 h治疗组(309.5±146.9 m L)、(418.2±191.9 m L)及对照组(175.2±118.1 m L)、(233.8±195.3 m L)(P<0.05)。结论:按摩配合穴位注射对于预防混合痔术后尿潴留具有良好的疗效,并且提早干预能有效预防尿潴留的发生。Objective： To compare the efficacy difference in the different periods of massage with acupoint injection in the treat- ment of urinary retention after mixed hemorrhoid surgery. Methods： 120 case of urinary retention after mixed hemorrhoid surgery were randomized into a control group （40 cases）, postoperative 4h treatment group （40 cases） and postoperative 8h treatment group （40 cases） . All groups postoperative routine nursing were adopted, beside, the two treatment groups on the basis of routine nursing combined with massage and local acupoint injection in Zusanli （ST36） . Results： The effective rate was 92. 5% （37/40） in the postop- erative 4h treatment group, which was superior to 77. 5% （31/40） in the postoperative 8h treatment group and 55% （22/40） in the control group （ both P 〈 0. 05） . The first micturition time and the mean effect time were （ 33.6± 5. 8） rain and （57.4 ± 2. 2） min in the postoperative 4h treatment group separately, while （39. 5±4. 3） min and （68.9 ± 1.7） in the postoperative 8h treatment group, （63.3 ± 7.9） rain and （87.4± 5.6） min in the in the control group. The first micturition time and the mean effect time in the postoperative 4h treatment group were shorter apparently than those in the postoperative 8h treatment group and control group （ both P 〈0. 05） . The first urine output and 2 h urine output after treatment were （363.6 ± 158. 7） mL and （447.4 ± 198.2） mL in the postoperative 4 h treatment group separately, while （309.5 ± 146.9） min and （418.2 ± 191.9） in the postoperative 8h treatment group, （ 175.2± 118.1 ） min and （233.8 ±195.3） rain in the in the control group. The first urine output and 2h urine output after treatment in the postoperative 4h treatment group were much more apparently than those in the postoperative 8h treatment group and control group （ both P 〈 0. 05 ） .
