颈部增大淋巴结超声造影灌注特点及时间-强度曲线参数分析

摘　　要：目的分析颈部增大淋巴结超声造影灌注模式及时间-强度曲线(TIC)参数特点,探讨超声造影对颈部淋巴结的诊断价值。方法回顾性分析97例颈部增大淋巴结超声造影灌注模式及TIC参数特点,比较转移性淋巴结、淋巴瘤及反应增生性淋巴结超声造影表现及TIC参数。结果超声造影灌注模式中造影剂动脉期灌注方式(向心性强化/离心性强化)、是否均匀增强及是否合并无增强区在转移性淋巴结与反应增生性淋巴结之间、转移性淋巴结与淋巴瘤两组间比较差异均具有统计学意义(P<0.05)。转移性淋巴结峰值强度(PI)与淋巴瘤及反应增生性淋巴结差异均有统计学意义(P<0.05),TIC参数k在转移性淋巴结与淋巴瘤两组间比较差异具统计学意义(P<0.05)。结论超声造影增强模式及时间-强度曲线参数特点有助于提供诊断信息,对颈部增大淋巴结诊断及鉴别有一定价值。Objective To evaluate perfusion characteristics of contrast-enhanced ultrasound （CEUS） and parameters of time intensity curve （TIC） of enlarged cervical lymph nodes. Methods In a retrospective study, CEUS were performed in 97 patients with enlarged cervical lymph nodes. Perfusion characteristics of CEUS and parameters of TIC were analyzed. The CEUS findings were compared with the histology （metastatic lymph nodes, lymphoma, reactive lymph nodes）. Results Compared metastatic lymph nodes with lymphoma, metastatic lymph nodes with reactive lymph nodes, differences of CEUS perfusion characteristics including perfusion pattern （Centripetal enhancement/cen trifugal enhancement）, enhancement homogeneity and perfusion defects were markedly significant （P 〈 0. 05）; Differences of peak intensity （PI） between metastatic lymph nodes and reactive lymph nodes, metastatic lymph nodes and lymphoma were markedly significant（P〈0.05）. The difference of k between metastatic lymph nodes and lymphoma was statistically significant （P〈 0.05）. Conclusions Perfusion characteristics of contrast-enhanced ultrasound （CEUS） and parameters of time intensity curve （TIC） could provide more information for differential diagnosis of enlarged cervical lymph nodes.
