超声引导颈浅丛阻滞下颈内静脉置管的临床观察

摘　　要：目的探讨超声引导颈浅丛阻滞下颈内静脉置管在临床中的应用。方法 64例颈内静脉置管患者随机分为超声组(超声引导颈浅丛阻滞组)和对照组(局麻组),记录两组患者疼痛评分、舒适度评分和睡眠质量、焦虑评分及穿刺时间和并发症。结果与对照组相比,超声组颈内静脉穿刺后6、12、18h疼痛评分低、舒适度评分高,48h后睡眠质量、焦虑评分低(P<0.05),超声组的操作用时少及并发症发生率低(P<0.05)。结论超声引导颈浅丛阻滞下颈内静脉置管能提高留置舒适度,值得临床推广。Objective To investigate the application of superficial cervical plexus block in the internal jugular vein indwelling under ultrasound guidance. Methods 64 patients of internal jugular vein indwelling were randomly divided into ultrasound group （ultrasound-guided superficial cervical plexus bloCk） and control group （local anesthesia group）. Pain scores, comfort scores, sleep quality, anxiety scores, puncture time and complication were recorded. Results The pain score of the ultrasound group at 6, 12, 18 hours was lower than the control group （P〈0.05）, the comfort score was higher than the control group （P〈0.05）, and the sleep quality, anxiety score after 48 hours was lower than the control group （P〈0. 05）. The incidence of complications and the puncture time in the ultrasound group was lower than the control group （P〈0.05）. Conclusions Ultrasound guided cervical plexus block in the internal jugular vein puncture catheter can improve the indwelling Comfort, it is worth of clinical promotion.
