摘　　要：目的探讨超声在肝上皮样血管内皮瘤(HEHE)诊断中的价值。方法对29例经病理证实为HEHE患者的超声图像进行回顾性分析,研究肿瘤的大小、位置、回声、边界、内部结构以及彩色多普勒血流信号特点。结果 29例HEHE患者共132个病灶,直径范围1~6.8cm,高回声病灶25个,低回声病灶36个,稍低回声病灶66个,等回声病灶5个。81个病灶回声欠均匀,51个病灶内部回声尚均匀;52个病灶边界欠清,26个病灶边界不清,54个病灶边界尚清。10例患者较大病灶周边伴晕环,6例患者伴钙化,4例患者病灶之间互相融合,9例患者显示周边彩色血流信号,11例患者显示周边及内部均有彩色血流信号,18例动脉血流,2例静脉血流,平均阻力指数(RI)0.63±0.06。结论灰阶超声结合彩色多普勒超声对于肝脏上皮样血管内皮瘤的诊断具有一定的帮助。Objective To investigate the value of conventional ultrasound（US） in the diagnosis of hepatic epitheliold hemangioendothelioma（HEHE）. Methods The US images were retrospectively analyzed in 29 HEHE cases who were confirmed by pathology. The size, location, echo, boundary, the appearance of internal structures and color flow signals of hepatic lesions were observed by US. Results 132 lesions were shoved in 29 HEHE cases. The diameter range of hepatic lesions was from 1 to 6.8 cm. The tumors were displayed as hyperechoic lesions in 25 cases, hypoechoic lesions in 36 cases, slightly hypoechoic lesions in 66 cases, isoechoic lesions in 5 cases by conventional US. 81 lesions were showed heterogeneous while the other 51 homogeneous. Obscure,unclear and clear of the tumor boundaries were found in 52,26,54 lesions, respectively. The＂dark ring＂around tumors were appeared in major lesions in 10 cases. Calcifications were found in 6 cases and integration lesions were discovered in 4 cases. The color doppler flow imaging（CDFI）detected peripheral blood flow in 9 cases,peripheral and internal blood flow in 11 cases and no blood flow signal in 9 cases. The venous spectrum were detected in 2 cases and arterial spectrum were detected in 18 cases and the average resistant index were 0.63± 0.06. Conclusions Conventional ultrasonography combined with colordoppler ultrasonography has important significance in diagnosis of HEHE.
