摘　　要：目的评价超声引导穿刺微波消融治疗腹盆腔恶性肿瘤的可行性、安全性、有效性。方法回顾性分析与总结379例(604个病灶)腹盆腔恶性肿瘤患者接受微波消融治疗的临床资料,观察术后疗效、复发率及并发症的发生。结果 379例(604个病灶)腹盆腔恶性肿瘤患者在超声引导下穿刺微波消融治疗,手术过程顺利。消融时间180~720s,消融功率40~70 W。消融治疗1个月后超声造影或增强CT/MRI复查,完全消融549个病灶,部分消融55个病灶。肿瘤早期总灭活率为90.9%(549/604),总复发率为9.1%(55/604)。严重并发症发生率0.3%(2/604)。结论超声引导穿刺微波消融治疗腹盆腔恶性肿瘤是一种可行、安全、有效的方法,可成为一种微创治疗腹盆腔恶性肿瘤的姑息性治疗术,在临床上具有应用和推广价值。Objective To evaluate the feasibility, safety and efficacy of ultrasound guided percutaneous microwave ablation（UMA） in the treatment of malignant tumors of the abdominopelvic cavity. Methods The clinical data of 379 patients （604 lesions） who received microwave ablation for the treatment of abdominopelvic cavity malignant tumor were analyzed retrospectively, and the curative effect, recurrence rate and the incidence of complications were observed. Results In this group, 379 cases （604 lesions） were treated by ultrasound guided percutaneous microwave ablation in patients with abdominopelvic cavity malignant tumor. Ablation time was 180-720 s, ablation power was 40-70 W. 1 month after ablation treatment, contrast enhanced ultrasound or enhanced CT/MRI were performed as veview methods, complete ablation were showed in 549 lesions, partial ablation were showed in 55 lesions. The tumor early total inactivation rate was 90.9 % （549/604）, and the total recurrence rate was 9.1% （55/604）. The incidence of severe complications was 0.3%（2/604）. Conclusions Ultrasound guided percutaneous microwave ablation in the treatment of abdominopelvic cavity malignant tumors is a feasible, safe and effective method. It can become a kind of palliative surgery for minimally invasive treatment of abdominopeivic cavity malignant tumor and has the value of application and popularization in clinical practice.
