前列腺弹性成像Ⅲ级结节的超声造影研究

摘　　要：目的探讨超声造影(CEUS)经直肠超声弹性成像(TRE)在判定为Ⅲ级的前列腺结节中的临床使用价值,及结节深度对TRE和CEUS鉴别诊断该种前列腺结节的影响。方法对89个经TRE判定为Ⅲ级的前列腺结节按其距探头的深度(D)不同,分为A组(D<10mm)、B组(10mm≤D<20mm)及C组(D≥20mm),对上述3组结节进行CEUS并以靶向穿刺的病理结果为金标准,评价CEUS对该种前列腺结节的诊断价值,并评价结节深度对TRE及CEUS诊断该种结节的良恶性有无影响。结果 TRE的诊断符合率在A组与C组之间存在差异并具统计学意义(P<0.05);CEUS的诊断符合率在不同深度组之间均无统计学差异(P>0.05)。结论前列腺结节的深度影响TRE的诊断符合率;CEUS的临床应用有利于该种前列腺结节的进一步鉴别诊断。Objective To investigate the clinical value of contrast-enhanced ultrasound（CEUS） in differential diagnosis of the prostate nodules graded Ⅲ by transrectal elastography （TRE）, and the effect of prostate nodule depth on the diagnosis of TRE and CEUS. Methods According to the depth（D） from the surface of prostate nodules to the probe, 89 nodules detected by CEUS were divided in to three groups（Group A：D〈10 mm; Group B： 10 mm≤D〈20 mm;Group C： D≥20 mm）. The results of elastography-targeted pathologic biopsy were regarded as the golden standard to evaluate the diagnostic value of CEUS on different groups and to judge the effect of prostate nodule depth on the diagnosis of TRE and CEUS. Results The diagnostic accuracy of TRE had significant difference between Group A and Group C （P〈0.05） ,but that of CEUS had not statistical difference among 3 groups（P〉0.05）. Conclusions The depth of prostate nodules influenced the diagnostic accuracy of TRE, but the application of CEUS contributed to the differential diagnosis of prostate nodules graded Ⅲ by TRE.
