摘  要:目的探讨超声在少见桡神经损伤中的定位诊断价值。方法对7例上臂桡神经缩窄和5例骨间后神经损伤的患者进行超声检查,分析其超声声像图特征,并与临床外科手术进行对照。结果超声可清晰显示上肢桡神经束的走行及内部结构。12例患者中7例超声定位病变于上臂桡神经束,5例病变定位于前臂的骨间后神经,声像图表现为神经走行粗细不均,局部缩窄变细、两端神经束增粗,内部条索束膜结构消失,回声减低,外膜回声增强。超声定位和损伤处形态评估与术中发现一致。结论术前超声检查可明确上臂桡神经卡压及骨间后神经损伤,确定缩窄病灶位置及神经形态变化,对指导手术方案制定及评估预后有重要价值。Objective To evaluate localization diagnostic value of ultrasound in rare radial nerve injury. Methods 7 patients with upper arm radial nerve coarctation and 5 patients with posterior interosseous nerve lesion received high-frequency ultrasound examination and compared with their surgical findings. Results Ultrasound could show the distribution and inner structure of upper limb radial nerve branches. 7 of 12 cases lesions located in upper arm radial nerve, the other 5 cases of located in the fore-arm posterior interosseous nerve. The ultrasonographic appeared as asymmetrical thicker and thiner nerve trunk, regional eoarctation with peripheral nerve trunk enlargement, and lesion part showed as hypoechoic nodules with indistinct inner structure, and hyperechoic nerve epineurium. Lesion location and morphological assessment by ultrasound were accordance with surgical findings. Conclusions US is valuable in diagnosing upper limb radial nerve and fore arm posterior interosseous nerve lesion. It was helpful in lesion location and morphological assessment before operation, which is important for surgical therapy guidance and prognosis assessment.
