止晕三针治疗颈性眩晕经颅多普勒超声的随机对照研究

摘　　要：目的:探讨比较止晕三针与针刺、药物对颈性眩晕(CV)患者经颅多普勒超声(TCD)的影响。方法:将180例CV患者按简单随机法分为治疗组(针刺人迎穴、风池穴加额中线)、针刺组(针刺颈夹脊穴、风池、百会)和药物组(静滴川芎嗪加口服西比灵)各60例,经TCD检测CV患者治疗前后椎动脉(VA)和基底动脉(BA)的收缩期血流量(Vs)、平均血流量(Vm)、舒张期血流量(Vd)、搏动指数(PI)、阻力指数(RI)。结果:3组治疗后LVA和BA的Vs、Vm、Vd明显升高,PI、RI明显降低;治疗后治疗组RVA的Vs、Vm、Vd较治疗前明显升高,PI和RI较治疗前明显降低;治疗后治疗组RVA的Vs、Vm、Vd、PI与针刺组和药物组比较差异有统计学意义。结论:止晕三针在改善CV患者RVA的Vs、Vd、Vm和PI值方面均优于针刺和川芎嗪加西比灵的作用。Objective： To compare effect of 3 vertigo-stopping needles、ligustrazine combined with flunarizine、acupuncture on Transcranial Doppler（TCD） in patients with cervical vertigo. Methods： One hundred and eighty patients of cervical vertigo were allocated by simple randomization to the treatment group（3 vertigo-stopping needles group） 、acupuncture control group（acupuncture Renying point,Fengchi and frontal midline） and medication control group（ligustrazine combined with flunarizine） for 60 cases in each group. The systolic blood flow velocity（Vs）,diastolic blood flow velocity（Vd）,mean blood flow velocity（Vm）,pulsatile index（PI） and resistance index（RI） of basilar artery（BA）and vertebral artery（VA） were observed before and after treatmet by Transcranial Doppler（TCD）. Results： After treatment,the Vs,Vm,and Vd of BA and LVA were significantly increased,the PI and RI of BA and LVA were significantly reduced in both groups. The Vs,Vm,and Vd of RVA were significantly increased,the PI and RI of RVA were significantly reduced after intervention in treatment group. There were significant differences in comparing the Vs,Vm,Vd and PI of RVA between the treatment group and the other groups after treatment. Conclusion： 3 vertigo-stopping needles can improve the Vs,Vd,Vm and PI of Cervical Vertigo RVA. The improvement were better than the acupuncture treatment group and ligustrazine combined with flunarizine.
