摘  要:目的研究代谢手术对2型糖尿病患者认知功能的影响。方法选取2015年4月至2016年6月第三军医大学大坪医院住院的65例2型糖尿病患者,其中行代谢手术的患者30例,未行手术治疗的患者35例,分别完成体检、实验室检查、认知功能量表评估,并对两组患者一般资料及认知功能进行统计分析。结果在校正两组患者一般资料之前,在总体认知功能评价、记忆力与执行力、语言流畅性、视觉空间能力方面,手术组优于非手术组(P<0.05),在注意力与定向力方面,两组差异无统计学意义(P>0.05);在校正两组患者一般资料之后,在记忆力、执行力、语言流畅性方面,手术组优于非手术组(P<0.05);在总体认知功能评价、注意力与定向力、视觉空间能力方面,两组差异无统计学意义(P>0.05)。两组患者的认知功能与体重指数、血压、血糖、血脂及高敏C反应蛋白无相关性。结论代谢手术能改善糖尿病患者认知功能,且其机制可能独立于糖尿病患者体重、血压、糖脂代谢的改善。Objective To study the effect of metabolic surgery on the cognitive function in patients with type 2 diabetes. Methods Totally 65 patients with type 2 diabetes between April 2015 and June 2015 were included in this study. Among them, 30 patients were treated by gastric bypass metabolic surgery, while 35 patients received routine medication. All of them completed the physical examination, laboratory tests, and assessment of cognitive function scale, as well as evaluation. Results Before adjusting the related parameters of two groups, with regard to the overall evaluation of cognitive function, memory and execution and visual spatial ability, the surgery group was better than the non-surgery group(P〈0.05); however, attention and the directional force between two groups were not different (P〉0.05). After adjusting the related parameters, the memory, executive force, and language fluency were improved in surgical patients, but these were absent in non-surgical patients (P〈0.05). The overall evaluation of cognitive function, directional force, visual spatial attention and abihty between two groups were not significantly different after adjustment(P〉 0.05). Interestingly, the changes of cognitive functions were not correlated to BMI, blood pressure, blood glucose, blood lipid and high sensitive2CRP in diabetic patients. Conclusion The metabolic surgery can remarkably improve cognitive function in patients with diabetes, the mechanism of which may be independent of the body weight, blood pressure and glucose and lipid metabolism in diabetic patients.
