摘  要:目的:研究10种单味中药对临床分离的鲍曼不动杆菌的抗菌作用和对主动外排机制的抑制作用。方法:收集2011年1月至2011年12月宁波市第一医院患者临床标本中分离到的20株鲍曼不动杆菌,按照耐药性把20株菌分为多重耐药组和敏感组,每组10株。先检测10种单味中药对鲍曼不动杆菌的抑菌作用,再分别用外排抑制剂氰氯苯腙(终浓度15 mg/L)、利血平(终浓度20 mg/L)检测四环素和氯霉素外排泵表型,抗生素MIC值比空白对照下降4倍或以上时判为外排泵表型阳性。然后用终浓度2 000g/L的10种单味中药做外排抑制剂,检测四环素和氯霉素MIC值是否比空白对照下降4倍或以上。最后对数据进行统计分析。结果:10种单味中药对鲍曼不动杆菌都有不同程度的抑菌作用,黄岑和连翘的抑菌作用最强,金银花、三叶青、大青叶、黄连、野菊花的抑菌作用稍弱;焦栀子、蒲公英、紫花地丁的抑菌作用较弱。金银花、连翘、三叶青、焦栀子、大青叶、野菊花、蒲公英这7味中药对两组菌的抑菌作用有统计学差异(P<0.05):对敏感组菌的MIC_(50)和MIC_(90)都明显低于多重耐药组,而黄连、紫花地丁、黄岑对两组菌的抑菌作用无显著差异(P>0.05)。两组鲍曼不动杆菌对四环素和氯霉素的外排泵表型阳性率高,终浓度2 000 g/L的10种单味中药对氯霉素和四环素没有外排抑制作用。重新配制并提高本试验中10种单味中药水煎液浓度至终浓度为20 000 g/L,对氯霉素和四环素仍然没有外排抑制作用。结论:中药对鲍曼不动杆菌有抑菌作用,可作为抗生素的辅助用药。主动外排机制是鲍曼不动杆菌对四环素和氯霉素耐药的一种重要机制,但在20 000 g/L的浓度下,10种单味中药对氯霉素和四环素没有外排抑制作用。AIM: To investigate the antibacterial effect on Acinetobacter baumannii( A. baumannii) and to investigate the inhibitory effect on strains' active efflux mechanism of ten herbal medicines.METHODS: From January to December 2011,20 strains of A. baumannii were collected from Ningbo First Hospital and were divided into multidrug resistant group and more sensitive group according to the resistance,10 in each group. Antibacterial effect of ten herbal medicines on A. baumannii were performed,efflux pump phenotypes of tetracycline or chloramphenicol were performed by efflux inhibitor CCCP( final concentration: 15 mg / L) and reserpine( final concentration: 20 mg /L). Phenotypes were positive if MIC values of tetracycline or chloramphenicol decreased 4 times or more,compared with the blank control. And the final concentration of2 000 g / L of 10 kinds of single Chinese traditional medicine were used as efflux inhibitors to detect whether MIC values of tetracycline or chloramphenicol decreased 4 times or more,compared with the blank control. Finally,the data were statistically analyzed. RESULTS: Antibacterial effect of ten herbal medicines on A. baumannii have different degrees: Skullcap and Forsythia had the highest inhibitory effects; Honeysuckle,Clover,Folium Isatidis,Rhizoma Coptidis, wild Chrysanthemum showed weaker inhibitory effects; while antibacterial effect of Coke gardenia, Dandelion, Herba Violae was weak. Antibacterial effect of seven herbal medicines showed significant difference( P〈0. 05) between two groups: Honeysuckle, Forsythia, Radix Tetrastigmae,Fructus Gardeniae praeparatus,Folium Isatidis, wild Chrysanthemum, Dandelion. MIC50 and MIC90 of more sensitive group are significantly lower than multidrug resistant group, but antibacterial effect of Coptis,Violet,Skullcap showed no significant difference( P〈0. 05).
