摘  要:目的评价鞘内或切口局部A型肉毒素预先给药对切口痛大鼠脊髓背角神经激肽-1(NK-1)受体内化的影响。方法雄性SD大鼠,体重280~300 g,6~8周龄。实验Ⅰ 取鞘内置管成功7 d后无神经损伤表现的大鼠27只,采用随机数字表法分为3组(n=9):对照组(C1组)、切口痛组(IP1组)和鞘内注射A型肉毒素组(BoNT/A1组)。术前24 h时BoNT/A1组鞘内注射A型肉毒素0.5 U(溶于10 μl生理盐水中),IP1组鞘内注射生理盐水10 μl。实验Ⅱ 取27只大鼠,采用随机数字表法分为3组(n=9):对照组(C2组)、切口痛组(IP2组)和切口局部注射A型肉毒素组(BoNT/A2组)。术前24 h时,BoNT/A2组于切口局部皮下及跖肌注射A型肉毒素2 U(溶于0.4 ml生理盐水中),IP2组于切口局部皮下及跖肌注射生理盐水0.4 ml。每组取6只大鼠,分别于给药前、术前、术后3 h、1、3、5和7 d时测定右后足累计疼痛评分(CPS)和机械缩足反应阈(MWT)。术后3 h时每组取3只大鼠,采用免疫荧光法测定脊髓背角NK-1受体的表达水平。结果实验Ⅰ 与C1组比较,IP1组术后3 h、1、3、5和7 d时CPS评分升高,术后3 h、1和3 d MWT降低,脊髓背角NK-1受体表达上调,BoNT/A1组术后3 h、1、3和5 d时CPS评分升高,术后3 h时MWT降低(P〈0.05),脊髓背角NK-1受体表达差异无统计学意义(P〉0.05);与IP1组比较,BoNT/A1组术后3 h、1、3和5 d时CPS评分降低,MWT升高,脊髓背角NK-1受体表达下调(P〈0.05)。实验Ⅱ 与C2组比较,IP2组术后3 h、1、3、5和7 d时CPS评分升高,术后3 h、1、3和5 d时MWT降低,脊髓背角NK-1受体表达上调,BoNT/A2组术后3 h、1、3、5和7 d时CPS评分升高,术后3 h时MWT降低(P〈0.05),脊髓背角NK-1受体表达差异无统计学意义(P〉0.05);与IP2组比较,BoNT/A2组术后3 h、1、3和5 d时CPS评分降低,术后3 h、1和3 d时MWT升高,脊髓背角ObjectiveTo evaluate the effect of pretreatment with botulinum toxin A injected intrathecally or locally at the incision site on the neurokinin-1 (NK-1) receptor internalization in the spinal dorsal horn in a rat model of incisional pain.MethodsMale Sprague-Dawley rats, weighing 280-300 g, aged 6-8 weeks, were used in the study.The experiment was performed in two parts.Experiment Ⅰ Twenty-seven rats with no sign of nerve injury at day 7 after successful catheterization were selected and divided into 3 groups (n=9 each) using a random number table: control group (C1 group), incisional pain group (IP1 group) and intrathecal botulinum toxin A group (BoNT/A1 group). At 24 h before operation, botulinum toxin A 0.5 U (in 10 μl of normal saline) was injected intrathecally in group BoNT/A1, and normal saline 10 μl was injected intrathecally in group IP1.Experiment Ⅱ Twenty-seven rats were selected and divided into 3 groups (n=9 each) using a random number table: control group (group C2), incisional pain group (IP2 group) and locally injected botulinum toxin A at the incision site group (BoNT/A2 group). At 24 h before operation, botulinum toxin A 2 U (in 0.4 ml of normal saline) was injected subcutaneously at the incision site and into the plantar surface, and normal saline 0.4 ml was injected subcutaneously at the incision site and into the plantar surface in group IP2.Six rats in each group were selected, and the cumulative pain score (CPS) was recorded, and the mechanical paw withdrawal threshold (MWT) in the right hindpaw was measured before administration, before operation, and at 3 h and 1, 3, 5 and 7 days after operation.At 3 h after operation, 3 rats in each group were selected and sacrificed, and the lumbar segment (L4, 5) of the spinal cord was removed for determination of the expression of NK-1 receptors in the spinal dorsal horn by immunofluorescence.ResultsExperiment Ⅰ Compared with group C1, the CPS was significantly increased at 3 h and 1,
