强直性脊柱炎的炎性指标与骶髂关节炎X线分级的相关性

摘　　要：目的:探讨血沉(ESR)、C-反应蛋白(CRP)等炎性指标对强直性脊柱炎的病情程度和活动度的诊断价值。方法:收集129例临床诊断为AS的患者,测定ESR、CRP水平并拍摄骨盆平片诊断骶髂关节炎分级,运用单因素方差分析和Spearman相关分析方法进行数据分析。结果:(1)骶髂关节炎的X线分级Ⅲ级的患者的CRP水平显著高于0级、Ⅰ级的CRP水平;(2)骶髂关节炎的X线分级Ⅳ级的患者ESR显著高于X线分级0级、Ⅰ级的ESR水平;(3)ESR和CRP与骶髂关节X线分级均存在正相关关系。结论:ESR和CRP都对强直性脊柱炎活动性有一定的参考价值,可结合骶髂关节影像学检查综合判断,尽早调整治疗方案,控制疾病进展。Objective To explore the clinical value of the level of inflammatory factors （ESR, CRP. etc） on ankylosing spondylitis （AS）. Methods 129 patients with AS were enrolled. The stage of sacroiliitis was detected by pelvis X-ray and level of erythrocyte sedimentation rate （ESR） and C-reactive protein （CRP） were measured. Statistical analysis was performed with one way ANOVA and Spearman correlation analysis. Results （ 1 ）The level of CRP of patients with X-ray stage 11I sacroiliitis was significantly higher than patients with stage 0, I sacroiliitis （P 〈 0.05）; （2） The level of ESR of patients with X-ray stage 1Vsaeroiliitis was significantly higher than patients with stage 0, I sacroiliitis （P 〈 0.05） ; （3） The level of ESR and CRP both were positively correlated to the stage of sacroiliitis under X-ray. Conclusions The level of ESR and CRP may both contribute to evaluate the activity of AS. It can be combined with articulationes saeroiliaea imaging examination in formulating treatment plan and control disease progression.
