Treg细胞在类风湿关节炎合并潜伏性结核感染患者中的表达及意义

摘　　要：目的探讨类风湿关节炎合并潜伏性结核感染患者Treg细胞的表达及临床意义。方法将2015年1月至2016年1月于本院诊断为潜伏性结核感染的类风湿关节炎患者30例纳入观察组,并将本院同期无结核感染的类风湿关节炎患者30例纳入对照组。检测两组患者血清中Treg细胞及相关炎性因子水平,探讨Treg细胞在潜伏性结核感染中的临床意义。结果观察组患者Treg细胞水平[(4.36±1.12)%]明显低于对照组[(5.46±1.62)%](P<0.05),而Th17细胞和Th17/Treg细胞水平[(0.21±0.04)%,(0.25±0.07)%]均高于对照组[(1.01±0.19)%,(0.21±0.04)%](P<0.05)。观察组患者白介素(IL)-17水平[(19.65±2.36)pg/ml]明显高于对照组[(16.84±3.12)pg/ml](P<0.05),而IL-10和肿瘤坏死因子-α(TNF-α)水平[(4.93±0.42)pg/ml,(3.98±0.36)ng/ml]均明显低于对照组[(6.37±1.35)pg/ml,(4.93±0.42)ng/ml](P<0.05)。Pearson相关性分析显示,两组患者Treg细胞水平与Th17细胞和IL-17水平均呈负相关(r分别为-0.32、-0.54,P<0.05),而Treg细胞水平与IL-10和TNF-α水平均呈正相关(r分别为0.47、0.61,P<0.05)。结论类风湿关节炎合并潜伏性结核感染患者Treg细胞水平下降,Treg细胞水平与结核感染密切相关。Objective To investigate the effect of Treg cells in patients with rheumatoid arthritis combined with latent tuberculosis infection. Method 30 patients with rheumatoid arthritis combined with latent tuberculosis infection in our hospital from January 2015 to January 2016 were selected into observation group; in the same period, 30 cases ofrheumatoid arthritis patients without tuberculosis infection were brought into control group, the content of Treg cells and related inflammatory factors in the serum of the two groups were detected, and the significance of Treg cells in latent tuberculosis infection were explored. Result Treg Cell level of observation group [（4.36 ±1.12） %] was significantly lower than that of control group [（5.46± 1.62） %] （P 〈 0.05）, Thl7/Treg cell and Yhl 7 cell levels of observation group [（0.21± 0.04） %, （0.25±0.07） %] were higher than that of control group [（1.01±0.19） %, （0.21 ±0.04） %] （P 〈 0.05）. IL-17 level of observation group [（19.65±2.36） pg/ml] was higher than that of control group [（16.84±3.12） pg/ml] （P 〈 0.05）, IL- l0 and TNF-α levels of observation group [（4.93 ±0.42） pg/ml, （3.98±0.36） ng/ml] were significantly lower than that of control group [（6.37±1.35） pg/ml, （4.93 ±0.42） ng/ml] （P 〈 0.05）. Pearson correlation analysis showed that Treg cell levels of the two groups were negatively correlated with Th17 cells and IL-17 levels （r= -0.32, -0.54, P 〈 0.05）, and Treg cell levels of the two groups were positively correlated with IL-10 and TNF-α level （r = 0.47, 0.61, P 〈 0.05）. Conclusion The Treg cell level in patients with rheumatoid arthritis combined with latent tuberculosis infection is decreased, and Treg cell level is closely related to the tuberculosis infection.
