摘  要:目的:比较手足口病患者咽拭、肛拭标本中肠道病毒检出率是否有差异,了解不同的标本采集时间的肠道病毒检出率。方法:对许昌市2012年的临床诊断手足口病例350例分别采集咽拭和肛拭标本,进行荧光定量PCR检测柯萨奇病毒A 16型(Cox A16)、肠道病毒71型(EV71)和肠道病毒属其它病毒(EV)。同时根据350对咽拭和肛拭标本荧光定量PCR检测结果按照标本采集时间进行分析。结果:用x2检验比较两种标本检测结果的差异,EV71、EV和COXA16在咽拭子和肛拭子两种标本检测结果有差异,P值均<0.05。应用Kappa检验,Kappa值均>0.5,显示咽拭子和肛拭子的检测结果基本一致。根据配对标本检测EV71、EV和Cox A16结果与标本采集时间进行分析,对于EV71来说,咽拭标本在发病后3天、4~11天均可检出阳性,而Cox A16、其他肠道病毒在发病3天后都无法检出阳性。结论:肛拭标本总体阳性率高于咽拭标本。EV 71在呼吸道带毒持续时间长于Cox A16和其它肠道病毒属病毒。肠道病毒可能在发病2天内在咽部大量繁殖扩增,此时通过呼吸道传播的可能性较大。Objective: compared patients with HFMDenterovirus,pharyngeal swab anal swab specimens waswhether there was a difference. Understand the enteric virus specimen collection time of different detection rate.. method: the clinical diagnosis of the city of Xuchang in 2012 350 cases of HFMD were broken were collected from throat swab and rectal swab specimens,detection of Coxsackie virus A fluorescence quantitative PCR type16( Cox A16),enterovirus71( EV71) and enterovirus other virus( EV). At the same time,according to the 350 pharyngeal swabs and anal swabs fluorescence quantitative PCR detection results according to the time of sample collection,analysis. Results: difference between the 2 test was used to compare the two results,EV71,EV and Cox A16 in throat swabs and anal swabs of two samples have different results,P〈0. 05. Application of Kappa test,Kappa value 〉0. 5,show the pharynx swabs and analswabs were basically identical. According to the pairedsamples EV71,EV and Cox A16 results with the time of sample collection were analyzed,for EV71,throat swabspecimens in 3 days after the onset,4 ~ 11 days can bepositive,while Cox A16,Other enterovirus in 3 days after onset were notpositive. Conclusion:anal swab specimens of total positive rate is higher than the pharynx swabs. EV 71 inrespiratory poison duration is longer than the Cox A16 and enterovirus virus. Intestinal virus may at the onset within 2 days of pharyngeal blooms amplification,this time through the possibility of greater respiratory genetic sowing.
