摘 要:目的评价乙肝疫苗和乙肝免疫球蛋白联合免疫对乙肝病毒母婴传播的阻断效果。方法从张家港市设有产科的所有医院选取母亲为乙肝病毒表面抗原(HBs Ag)阳性且已接种乙肝疫苗和乙肝免疫球蛋白的婴幼儿512人,随访调查母亲乙肝病毒感染情况,分娩方式等,并检测婴幼儿血清中HBs Ag和乙肝病毒表面抗体(HBs Ab)水平。结果调查婴幼儿中,HBs Ag阳性率为1.76%,HBs Ab阳性率为87.11%。不同性别、不同分娩方式对婴幼儿乙肝感染率的影响差异无统计学意义。≤12月龄、12月龄~和≥24月龄的婴幼儿HBs Ab阳性率分别为100.00%、90.91%和84.62%。结论乙肝疫苗和乙肝免疫球蛋白阻断HBs Ag阳性母亲垂直传播乙肝病毒效果显著,婴幼儿体液免疫水平随月龄增加而逐步降低。Objective The aim of this study was to evaluate blocking effect of Hepatitis B vaccine and HBIG on the mother- infant transmission of Heptitis B. Methods A total of 512 babies born with positive hepatitis B surface antigen (HBsAg) were selected as the study subjects from all of the hospitals with the obstetrical ward in Zhangjiagang city. They were inoculated with HBIG and hepatitis B vaccine. The maternal infection situation of hepatitis B and the baby delivery method were surveyed. The infants' venous blood was collected to test the level of HBsAg and HBsAb in serum. Results The positive rates of HBsAg and HBsAb in infants were 1.76% and 87.10%, respectively. The positive rate of HBsAg in infants did not show any statistical difference between different genders. Different delivery method did not significantly affect the infection rate of hepatitis B. The positive rates of HBsAb were 100%, 90.91% and 84.62%, respectively in ≤ 12, 12 above and ≥24 months old infants. Conclusion The blocking effect of HBIG in combination with hepatitis B vaccine on the mother-infant transmission of hepatitis B is obvious. Humoral immunity against B virus in infants was attenuated with increase of age.
