关节镜下保留前交叉韧带残端单隧道重建对手术效果的影响  

Surgery Effect of Single-tunnel Reconstruction with Preserving the Stump of Anterior Cruciate Ligament under Arthroscopy

摘　　要：目的:观察关节镜下保留前交叉韧带残端单隧道重建前交叉韧带的手术效果。方法:选取2014年1月至2015年3月我科行单隧道前交叉韧带重建术的24例患者,术中随机选择是否保留残端将其分为残端保留组和残端切除组,各12例。观察记录2组患者术后膝关节的Lysholm评分、IKDC评分。结果:残端保留组患者术后3、6个月Lysholm评分分别为(75.08±0.79)分和(84.67±1.16)分,IKDC评分分别为(77.33±1.37)分和(85.17±1.12)分;残端切除组患者术后3、6个月Lysholm评分分别为(71.42±1.44)分和(78.25±2.09)分,IKDC评分分别为(71.08±0.79)分和(79.25±2.09)分,差异均有统计学意义(P<0.05)。残端保留组患者术后9、12个月Lysholm评分分别为(86.75±1.22)分和(90.17±1.03)分,IKDC评分分别为(86.75±0.97)分和(89.67±1.37)分;残端切除组Lysholm评分分别为(85.92±1.16)分和(89.33±1.07)分,IKDC评分分别为(85.92±1.08)分和(88.50±1.24)分,差异均无统计学意义(P>0.05)。结论:保留前交叉韧带残端单隧道重建术能有利于更早地恢复膝关节功能。Objective： To observe the surgery effect of single-tunnel reeonstruction with preserving the stump of anterior erueiate ligament （ACL） under arthroseoty. Methods： The clinical data of 24 eases of ACL rupture undergoing single-tunnel anterior cruciate ligament reconstruction under arthroseopy from Jan 2014 to Mar 2015 were analyzed. The patients were divided into preserved ligament stump group （n=12）, and removed ligament stump group （n=12）, according to randomly choose whether the stump of ACL being preserved. Lysholm score and IKDC score were recorded after surgery of the two groups. Results： The Lysholm score was （75.08±0.79）, （84.67±1.16）） and IKDC score was （77.33±1.37）, （85.17±1.12） in preserved ligament stump group at 3, 6 months after surgery respectively, which were higher than those in removed ligament stump group [Lysholm score （ （71.42±1.44）, （78.25±2.09）） and IKDC score （（71.08±0.79）, （79.25±2.09） ] （P〈0.05） . However, there was no difference in Lysholm score and IKDC score at 9, 12 months after surgery （P 〉 0.05） . Conclusion： Single-tunnel reconstruction with preserving the stump of ACL can help restore knee function sooner.
