隔日脑脊液限压引流在小儿化脓性脑膜炎中的应用

摘　　要：目的探讨对化脓性脑膜炎患儿采用隔日脑脊液限压引流的临床效果。方法选取我院2014年10月至2015年9月收治的化脓性脑膜炎患儿100例,随机分为对照组和观察组,每组50例。观察组采用隔日脑脊液限压引流疗法,对照组采用萘夫西林与美罗培南联合疗法。治疗1周后,比较两组患儿的治疗效果、实验室指标和临床症状改善情况。结果观察组的总有效率为88.0%,显著高于对照组的64.0%,差异具有统计学意义(P<0.05)。治疗后,观察组的外周白细胞恢复时间、GSF白细胞恢复时间、颅高压消失、止惊、嗜睡和昏迷改善时间均显著少于对照组,差异均具有统计学意义(P<0.05)。结论采用隔日脑脊液限压引流法治疗小儿化脓性脑膜炎,可显著改善患儿的临床症状及实验室指标,临床效果确切,值得临床推广及应用。Objective To explore the clinical effect of alternate cerebrospinal fluid pressure limiting and drainage in children withpurulent meningitis. Methods 100 cases of children with purulent meningitis admitted to our hospital from October 2014 to September 2015were selected and randomly divided into control group and observation group, with 50 cases in each group. The observation group receivedcerebrospinal fluid pressure limiting and drainage, while the control group was treated with naphthalene and meropenem. After one-weektreatment, the treatment effect, laboratory indicators and improvement of clinical symptoms of children were compared between two groups.Results The total effective rate of observation group was 88.0%, significantly higher than 64.0% of control group, with statistical difference（P 〈0.05）. After treatment, the recovery time of peripheral white blood cells and GSF leukocyte, the improvement time of cranial pressuredisappearance, convulsion control, drowsiness and coma of observation group were significantly less than those of control group, withstatistical differences （P〈0.05）. Conclusions Alternate cerebrospinal fluid pressure limiting and drainage in the treatment of children withpurulent meningitis can significantly improve the clinical symptoms and laboratory indicators of children, with exact clinical effect, which isworthy of clinical promotion and application.
