加味参苓白术散口服联合白细胞介素-2腹腔灌注治疗卵巢癌相关腹水的临床观察

摘　　要：【目的】探讨加味参苓白术散口服联合白细胞介素-2(IL-2)腹腔灌注治疗卵巢癌相关腹水的临床效用。【方法】将60例卵巢癌腹水患者随机分为治疗组和对照组各30例。治疗组以IL-2腹腔灌注并口服中药汤剂加味参苓白术散,对照组单纯给予IL-2腹腔灌注,观察2组腹水控制情况、中医证候评分及Karnofsky量表评分(KPS),评估2组临床作用。【结果】(1)治疗后,治疗组腹水控制有效率为86.7%,优于对照组的53.3%,差异有统计学意义(P<0.05)。(2)治疗后,治疗组各项中医证候评分均较治疗前明显改善(P<0.05);而对照组仅腹部坚满、青筋显露、食少纳差、腹痛绵绵、水肿、小便短少等证候评分有改善(P<0.05);组间比较,治疗组在改善腹部坚满、神疲懒言、体倦乏力、水肿、小便短少、大便稀薄等证候评分方面明显优于对照组,差异有统计学意义(P<0.05)。(3)治疗后,治疗组KPS评分呈上升趋势,而对照组呈下降趋势,与治疗前比较,差异均有统计学意义(P<0.05);组间比较,治疗组在改善KPS评分方面优于对照组(P<0.01)。【结论】加味参苓白术散联合IL-2腹腔灌注治疗卵巢癌相关腹水,可以有效控制腹水情况,改善症状,提高生活质量。Objective To investigate the clinical effect of oral use of modified Shenling Baizhu Powder combined with intraperitoneal perfusion of interleukin-2（IL-2）for the treatment of ascites of ovarian cancer. Methods A total of 60 cases of ovarian cancer ascites were randomized into treatment group and control group,30 cases in each group. The treatment group was treated with intraperitoneal perfusion of IL-2 and oral use of modified Shenling Baizhu Powder, and the control group was only given intraperitoneal perfusion of IL-2. The relief of ascites,traditional Chinese medical syndroms scores, and Karnofsky Performance Status（KPS）scores were observed for the evaluation of the clinical effect. Results（1）After treatment,the effective rate for controlling ascites of the treatment group was 86.7%, and that of the control group was 53.3%, the difference being statistically significant（P 0.05）.（2）After treatment, scores of all of the traditional Chinese medical syndromes of the treatment group were markedly improved（P 0.05）,while the control group had an effect only on improving the scores of abdominal distention and rigidity, prominent veins, poor appetite, continuous and dull abdominal pain,edema,and oliguria（P 0.05）. The treatment group had better effect than the control group on improving the scores of abdominal distention and rigidity,lassitude and no desire to speak,weakness and fatigue,edema,oliguria,and loose stool（P 0.05）.（3） After treatment, KPS scores were increased in the treatment group and were decreased in the control group（P 0.05 compared with those before treatment）,and the effect on improving KPS scores in the treatment group was superior to that in the control group（P 0.01）. Conclusion Modified Shenling Baizhu Powder combined with IL-2 intraperitoneal perfusion is effective for the treatment of ovarian cancer ascites,and can effectively control ascites,relieve the symptoms and signs,and improve the quality of life.
