Embosphere联合PVA介入栓塞治疗大咯血45例疗效观察

摘　　要：目的:探讨用Embosphere联合PVA行选择性支气管动脉栓塞术治疗大咯血的效果。方法:45例患者均采用Seldinger法介入栓塞治疗大咯血,选择适当直径的Embosphere及PVA联合栓塞支气管动脉及相关病变血管。结果:45例患者治疗后均完全止血,有效率为100%,随访1~5年3例复发,咯血量和次数均较前明显减少,复发率为6.7%,行再次介入栓塞后均无再咯血;术中2例出现胸痛,术后3例出现发热、咳嗽等症状,经过对症处理后,均在2~4d内缓解,无肢体瘫痪等严重并发症发生。结论:Embosphere联合PVA行选择性支气管动脉栓塞治疗大咯血疗效显著,复发率低,并发症少,是治疗大咯血首选。Objective：To evaluate the effect of Embosphere combined with PVA performing interventional embolization in serious hemoptysis treatment.Methods：By using seldinger interventional embolization in all 45 cases serious hemoptysis,choose appropriate diameter Embosphere and PVA combined embolization of bronchial artery and related leions.Results：After treatment of 45 cases were completely stop bleeding,effective rate was 100%,and were followed up for 1~5years 3cases relapse,hemoptysis quantity and times of all the previous decreased significantly,the recurrence rate was 6.7%,line again,no longer hemoptysis after interventional embolization;in 2cases with chest pain,3cases of postoperative fever,the symptom such as cough,after symptomatic treatment,all cases recover in 2~4days,no paralysis and other serious complications.Conclusion：Embosphere combined with PVA performing selective bronchia rtery embolism in the serious hemoptysis as obvious therapeutic effect,low recurrencerate,less complications,is the preferred alternative treatment for Serious hemoptysis.
