热疗联合腹腔内灌注化疗治疗腹腔恶性肿瘤并腹腔积液的疗效观察

摘　　要：目的评价单纯腹腔内灌注化疗以及联合热疗两种方案治疗晚期腹腔恶性肿瘤并腹腔积液的近期疗效与毒副反应。方法收集具有完整病例资料的腹腔恶性肿瘤并腹腔积液患者127例,按治疗方法分为单纯腹腔灌注化疗组59例与热疗联合腹腔灌注化疗组68例。比较两组近期疗效、生活质量、药物毒副反应。结果热疗联合腹腔灌注化疗组治疗总有效率55.88%,明显高于单纯腹腔灌注化疗组的33.90%,差异有统计学意义(χ~2=6.460,P=0.011)。热疗联合腹腔灌注化疗组患者生活质量明显改善例数显著高于单纯腹腔灌注化疗组(χ~2=5.530,P=0.019)。两组患者的消化道反应、骨髓抑制、肝肾功损害的发生率比较,差异无统计学意义(P>0.05)。结论热疗联合腹腔灌注化疗治疗晚期腹腔恶性肿瘤安全有效,临床上值得推荐给部分具有适应证患者。Objective To evaluate the clinical efficacy and toxic reaction of intraperitoeal perfusion chemotherapy combined with hyperthermia for the advanced abdominal cancer patients with malignant peritoneum effusion. Methods Complete information of 127 patients with advanced abdominal cancer combined with malignant peritoneum effusion were collected. They were divided into perfusion chemotherapy group （n = 59） and perfusion chemotherapy combined with hyperthermia group （n = 68 ）. Curative efficacy, survival quality and toxic reaction were compared between the two groups. Results For the perfusion chemotherapy combined with hyper- thermia group,the response rate （RR） were 55.88% that was significantly higher than that in the perfusion chemotherapy group （RR = 33.90% ） （X2 = 6. 460, P = 0. 011 ）. The quality of life （QOL） of the perfusion chemotherapy combined with hyperthermia group patients was significantly improved in 37 cases （54. 41% ） that was significantly higher than that in the perfusion chemotherapy group （ 19 patients ,32. 20% ） （X2 =5. 530 ,P = 0. 019）. There was no difference in the rate of digestive tract reaction,bone marrow inhibition and damage of liver and kidney function between the two groups. Conclusion Perfusion chemotherapy combined with hyperthermia for advanced abdominal cancer with ascites is effective and safe. It is worth to recommend for a part of patients.
