丹参注射液联合常规西药对心绞痛疗效的Meta分析

摘　　要：目的系统评价丹参注射液联合西药治疗心绞痛的效果。方法检索数据库PubMed、Cochrane Library、EMbase、Sino-Med、Google学术、万方数据库及中国知网中2000年1月至2016年1月发表的丹参注射液联合常规西药治疗心绞痛随机对照临床试验（randomized controlled trails,RCTs）,筛选符合纳入标准的文献,选择心绞痛症状疗效和心电图疗效作为效应指标,采用RevMan 5.0和Stata 12.0软件进行Meta分析。采用修订版Jadad评分量表评价纳入文献的方法学质量。结果共检索到1493篇文献,最终纳入12篇RCT,共计1194例患者。与单纯常规西药组（对照组）相比,丹参注射液联合常规西药组（治疗组）能够显著提高心绞痛的临床治疗效果（OR=4.16,95%CI：2.97-5.84,I2=0%,P〈0.001）,明显改善心绞痛的心电图表现（OR=2.60,95%CI：1.87-3.61,I2=0%,P=0.004）。仅有两篇文献报道出不良反应,在降低不良反应发生率方面,治疗组和对照组无显著性差异。结论丹参注射液联合常规西药可改善心绞痛的临床疗效及心电图表现。由于所纳样本量少、质量偏低,仍需严格设计的高质量、大样本的随机双盲对照试验来验证其临床疗效。Objective To review systematically the efficacy of Danshen Injection combining conventional Western drugs in treatment of angina pectoris. Methods The databases of Pub Med, Cochrane Library, EMbase, SinoMed, Google Scholar, Wan Fang Database and CNKI were retrieved for searching randomized controlled trails（RCT） about the treatment of angina pectoris with Danshen Injection combining conventional Western drugs published from Jan. 2000 to Jan. 2016, and the eligible literature to inclusive standard were screened. The curative effect on symptoms of angina pectoris and curative effect showed by electrocardiogram（ECG） were taken as effect indexes, and a Metaanalysis was conducted by using Rev Man 5.0 software and Stata 12.0 software. The methodological quality of inclusive literature was reviewed by using revised Jadad scale. Results There were totally 1493 literature retrieved and finally 12 RCT included involved 1194 patients. Compared with control group（with only conventional Western drugs）, the curative effect was significantly improved（OR=4.16, 95%CI： 2.97-5.84, I2=0%, P〈0.001）, and ECG manifestations were much better（OR=2.60, 95%CI： 1.87-3.61, I2=0%, P=0.004） in treatment group（with Danshen Injection combining conventional Western drugs. There were only 2 literature reported adverse reactions, and treatment group and control group had no significant difference. Conclusion Danshen Injection combining conventional Western drugs can improve the curative effect and ECG manifestations in the treatment of angina pectoris. As the samples are less and quality is lower in this study, more large-sample double-blinded RCT with stricter design and higher quality are needed for verifying the curative effect.
