改良透明帽辅助胃镜检查在Barrett食管诊断中的应用及护理

摘　　要：目的观察改良透明帽辅助胃镜检查在Barrett食管诊断中的应用效果,并总结其护理要点。方法将163例在本院内镜中心行胃镜活检术的患者按检查次序编号,采用2∶1方法分为常规组108例,改良组55例。常规组患者在胃镜活检术中采用常规透明帽,改良组采用改良透明帽,比较两组病理活检阳性率和不良反应发生率的差异。结果改良组患者病理活检阳性率(96.7%)高于常规组(79.6%),组间比较,差异有统计学意义(P<0.05);两组患者误吞和出血发生率比较,差异无统计学意义(均P>0.05)。结论采用改良透明帽进行胃镜活检术,可提高Barrett食管的病理活检阳性率,并且是安全的。同时做好心理护理、正确安装透明帽和加强不良反应的观察对提高胃镜活检术成功率具有重要的意义。Objective To explore the effect of modified transparent cap on the diagnosis of Barrett＇s esophagus and summarize the nursing points. Methods One hundred and sixty-three patients undergoing endoscopic biopsy were randomly divided into the conventional group（n=108） and the modified group（n=60）. Routine transparent caps were used in the endoscopic biopsy of the patients in the conventional group. The modified transparent caps were used in the modified group. The pathological biopsy and adverse reactions were observed. Result The positive rate of pathologic biopsy in the modified group was significantly higher than that in the conventional group（P〈0.05）.There was no significant difference in adverse effect between two grroups（P〉0.05）. Conclusions Gastric biopsy with modified transparent cap can increase the positive rate of Barrett＇s esophagus in a safe way. At the same time,psychological care, correct installation of transparent cap and strengthened observation of adverse reactions are all critical for improving the success of endoscopic biopsy.
