自制扣眼松紧带在气管套管固定中的应用

摘　　要：目的探讨自制扣眼松紧带在气管套管固定中的应用效果。方法将62例气管切开患者随机分为两组,两组均按气管切开术后护理常规护理,对照组30例在常规护理的基础上,采用传统的棉寸带固定气管导管,观察组32例采用自制扣眼固定气管导管。对比观察两组患者的舒适度、皮肤损伤程度、气管导管脱出发生率。结果观察组更换气管套管固定带共381次,对照组更换气管套管固定带共377次。两组患者舒适度比较差异有统计学意义(P<0.01),观察组患者的舒适程度优于对照组。两组患者皮肤损害程度比较差异有统计学意义(P<0.05),观察组皮损程度显著轻于对照组。观察组无气管套管脱管发生,对照组发生气管套管脱管7例,发生率为23.33%,两组比较差异有统计学意义(P<0.05),观察组气管套管脱管发生率显著低于对照组。结论扣眼松紧带取材方便,操作简便,可增加患者的舒适度,降低皮肤损伤发生与气管套管脱出的风险,经济实用,值得临床推广应用。Objective To evaluate application effect of self-made buttonhole elastic band in tracheal tube fixation.Meth- ods 62 patients who underwent tracheotomy were randomly divided into two groups, and both groups were given conventional nursing after tracheotomy.Based on the conventional nursing, control group（ 30 cases） were given traditional cotton inch band to fix tracheal tube,while observation group （32 cases） were given self-made buttonhole elastic band to fix the tracheal tube. And then, patients＇ comfort degree, the degree of skin lesions and the incidence of tracheal catheter prolapse of the two groups were compared and observed.Results The tracheal tube fixing band replaced with a total of 381 times in the observa- tion group,and the control group was replaced with a total of 377 times. Difference of patients＇ comfort degree of the two groups was statistically significant （P〈0.01）, and that of the observation group was superior to that of the control group. Difference of the degree of skin lesions of the two groups was statistically significant（P〈0.05）, and the observation group showed a lesser degree.No tracheal catheter prolapse was found in the observation group, while the tracheal catheter prolapse was found in 7 cases of the control group,incidence was 23.33%, difference was statistically significant（P〈0.05） ,the incidence of tracheal catheter prolapse of the observation group was significantly lower than that of the control group.Conclusion Buttonhole elastic band is convenient with simple operation, it can increase patients＇ comfort, reduce skin damage and risk of tracheal catheter prolapse.In addition, it is economical and practical, and thus is worthy of clinical application.
