2型糖尿病患者下肢动脉血管回声跟踪技术检测指标与血脂及血糖的相关性

摘　　要：目的应用血管回声跟踪技术检测2型糖尿病患者足背动脉及腘动脉弹性指标,探讨其与血脂及血糖的相关性。方法 2型糖尿病患者40例(观察组),体检健康者40例(对照组),检测2组空腹血糖(fasting plasma glucose,FPG)、糖化血红蛋白(glycosylated hemoglobin,HbA_(1c))、总胆固醇(total cholesterol,TC)、三酰甘油(triacylglycerol,TG)、高密度脂蛋白胆固醇(high density lipoprotein-cholesterol,HDL-C)、低密度脂蛋白胆固醇(low density lipoproteincholesterol,LDL-C)水平;应用血管回声跟踪技术检测2组足背动脉、腘动脉僵硬度(stiffness parameterβ,β)、血管压力应变弹性系数(pressure-strain elasticity coefficient,E_ρ)、顺应性(arterial compliance,AC)、脉搏波传导速度(pulse wave velocityβ,PWVβ);Pearson相关分析2型糖尿病患者足背动脉和腘动脉弹性指标与FPG、HbA_(1c)、TC、TG、HDL-C、LDL-C的相关性。结果观察组FPG[(8.01±1.95)mmol/L]、HbA_(1c)[(8.66±1.64)mmol/L]、TC[(5.28±0.84)mmol/L]、TG[(1.96±0.89)mmol/L]、LDL-C[(3.11±0.65)mmol/L]高于对照组[(5.35±0.62)、(5.19±0.29)、(4.77±0.21)、(1.54±0.11)、(2.82±0.30)mmol/L](P<0.05);观察组左、右侧足背动脉β(14.41±5.06,14.68±4.80)、E_ρ[(198.35±72.89)、(199.82±67.24)kPa]、PWVβ[(8.46±1.66)、(8.52±1.51)m/s]高于对照组[(10.99±4.28)、(11.37±4.25);(137.09±52.38)、(141.27±54.82)kPa;(7.05±1.37)、(7.18±1.37)m/s](P<0.05),AC值[(0.080±0.030)、(0.082±0.040)mm^2/kPa]低于对照组[(0.104±0.043)、(0.103±0.042)mm^2/kPa](P<0.05);观察组腘动脉左、右侧β(14.21±6.08、14.38±5.86)、E_ρ[(200.35±96.12)、(200.60±88.66)kPa]、PWVβ[(8.51±2.00)、(8.37±1.79)m/s]高于对照组[9.85±4.78、10.29±4.20;(122.77±63.00)、(127.15±52.10)kPa;(6.64±1.62)、(6.79±1.79)m/s](P<0.05),AC值[(0.30±0.14)、(0.34±0.18)mm^2/kPa]低于对照组[(0.52±0.22)、(0.49±0.20)mm^2/kPa](P<0.05);2型糖尿病患者足背动脉、腘动脉弹性指标β、E_ρ、PWVβ与FPG、HbA_(1c)、TC、LDL-C呈正�Objective To detect the elasticity indexes of dorsal artery of foot and popliteal artery by echo-tracking technique in patients with type 2 diabetes mellitus （T2DM） to analyze their correlation with blood lipid and blood glucose. Methods The levels of fasting plasma glucose （FPG）, glycosylated hemoglohin （HhA1c）, total cholesterol （TC）, triacylglycerol （TG）, high density lipoprotein-cholesterol （HDL-C） and low density lipoprotein-cholesterol （LDL-C） were detected in 40 patients with T2DM （observation group） and 40 healthy volunteers （control group）. Echo-tracking technique was used to measure the stiffness parameter β （β）, pressure-strain elasticity coefficient （Eρ）, arterial compliance （AC） and pulse wave velocity β （PWVI3） of dorsa artery of foot and popliteal artery. The correlation of the above elasticity indexes with FPG, HbA1c, TC, TG, HDL-C and LDL-C were studied by Pearson correlation analysis. Results The levels of FPG （（8. 014-1.95） retool/L）, HbAlc （（8.66±1.64） mmol/L）, TC （（5.28±0.84） retool/L）, TG （（1.96±0.89） mmol/L） and LDL-C （（3.11±0. 65） mmol/L） in observation group were significantly higher than those in control group （（5.35±0.62）, （5.19±0.29）, （4.77±0.21）, （1.54±0. 11）, （2.82±0.30） mmol/L） （P〈 0.05）. The values of 13 （14. 41±5.06, 14. 68±4. 80）, Eρ （（198. 35±72. 89）, （199. 82±67. 24） kPa） and PWVβ （（8.46±1.66）, （8.52±1.51） m/s） of left and right dorsal arteries in observation group were significantly higher than those in control group （10.99±4.28, 11.37±4.25, （137.09±52.38）, （141.27±54.82） kPa; （7.05±1.37）, （7.18± 1.37） m/s） （P〈0.05）, and AC values （（0. 080 ±0. 03）, （0. 082± 0. 04） mm2/kPa） in observation group were significantly lower than those in control group （（0. 104±0. 043）, （0. 103±0. 042） mm2/kPa） （P〈0.05）.
