海南省卫生资源配置公平性及其变化趋势

摘　　要：目的:分析海南省卫生资源配置公平性及其变化趋势,为相关部门合理制定区域卫生规划、提高卫生资源配置的公平性提供参考依据。方法:采用描述性分析、基尼系数和泰尔指数对卫生资源公平性进行评价。结果:2010—2014年,海南省主要卫生资源总量呈上升趋势,医疗机构数、医疗机构床位数、卫生人员数、卫生技术人员数、执业(助理)医师数、注册护士数增幅分别为121.00%、32.92%、36.97%、30.26%、26.12%和26.04%。与2010年相比,2014年每千人口医疗机构数、医疗机构床位数、卫生人员数、卫生技术人员数、执业(助理)医师数、注册护士数亦有明显增加。基尼系数和泰尔指数分析结果显示:(1)2014年海南省医疗机构配置的公平性最好,注册护士配置的公平性最差;(2)2010—2014年,海南省医疗机构配置公平性得到较大改善;医疗机构床位数、执业(助理)医师的公平性较为稳定;卫生人员和卫生技术人员的公平性有一定改善;注册护士的公平性无明显改善。结论:海南省在2009年后,卫生资源总量明显增加,主要卫生资源配置的公平性得到一定改善,但结构性问题有待进一步调整和优化。Objective： To analyze the distribution equality and trend of change of health resources in Hainan Province from 2010 to 2014. Methods： The equality of health resources was calculated by Gini coefficient and Theil index. Descriptive analysis was used. Results： During 2010 and 2014, the total number of health providers, beds, health workers, health technical personnel, licensed physicians or physicians assistants and registered nurses increased dramatically in Hainan Province. The rates of increase were 121.00%, 32.92%, 36.97%, 30.26%, 26.12% and 26.04%, respectively. Gini coefficient and Theil index results demonstrated that in 2014, the health provider allocation was the most equitable while the distribution equity of the registered nurse allocation was the worst. It also showed that the distribution equality of health providers was greatly improved, the distribution equality of beds, and licensed physicians or physician assistants was stable, and the distribution equity of health workers, health technical personnel was improved during the five years. However, the equality of registered nurses has not been improved during the five years. Conclusion： Total numbers of health resources increased and the overall equality of most of the major health resources was improved during the five years in Hainan Province after 2009. However, the composition of the health resources still needs improvement in the future.
