江西省公立医院门诊与住院病人医疗费用现状分析

摘　　要：目的:了解江西省公立医院门诊和住院病人医疗费用现状及其构成情况,为降低医疗费用提出相关政策建议。方法:通过江西省卫生计生委收集相关数据并做统计学描述,计算不同级别和主管单位的公立医院门诊与住院病人医疗费用的构成分布情况。结果:2014年江西省公立医院门诊病人人均医疗费用合计为183.9元,其中药费占51.7%;一、二、三级医院门诊病人人均医疗费用分别为114.4元、157.7元和232.1元;各级医院住院病人合计人均医疗费用为6 337.2元,其中药费占比44.4%,一、二、三级医院住院病人人均医疗费用分别为4 218.3元,4 280.0元和10 307.5元。结论:江西省公立医院门诊与住院病人医疗费用中药占比高,各级医院和县属至省属各层医院的门诊和住院病人人均医疗费用均呈现逐层递增情况,尤其是住院病人费用成倍剧增。建议政府应该积极采相关取措,推进分级诊疗,禁止不合理用药,规范医生和医院的诊疗行为。Objective： To understand the status of outpatients and inpatients＇ medical expenses among public hospitals in Jiangxi Province, and put forward some relevant policies and recommendations to reduce medical costs. Methods： Data was collected from Jiangxi Commission of Health and Family Planning. Descriptive statistics including frequencies and proportions were used to summarize the distribution of outpatients and inpatients＇ medical expenses among different levels and competent units in public hospitals. Results： In 2014, outpatients medical expenses amounted to 18.39 yuan per capita in public hospitals, which accounted for 51.7% of drugs. Outpatients medical expenses per capita of primary hospitals, secondary hospitals and tertiary hospitals accounted for 114.4 yuan, 157.7 yuan and 232.1 yuan, respectively; the total medical expenses per capita of all hospitals were 6 337.2 yuan, which accounted for 44.4% of drugs. Inpatients＇ medical expenses per capita of primary hospitals, secondary hospitals and tertiary hospitals accounted for 4 2 1 8.3 yuan, 4 280.0 yuan and 10 307.5 yuan, respectively. Conclusion： Drug proportion of outpatients and inpatients＇ medical expenses are high in public hospitals of Jiangxi Province. Outpatients and inpatients＇ medical expenses per capita increased by level among all levels of hospitals. Inpatients＇ medical expenses have especially multiplied. It is suggested that the government take measures to prohibit unreasonable prescription and regulate the doctors and clinics＇ behavior.
