摘　　要：目的探讨谷氨酰胺联合早期肠内营养治疗急性重症胰腺炎的临床疗效及对炎症因子的影响。方法选取86例急性重症胰腺炎患者随机分为观察组和对照组,分别给予谷氨酰胺联合早期肠内营养治疗和单纯早期肠内营养治疗,比较两组患者临床疗效、治疗前后肝肾功能指标、细胞炎症因子水平。结果观察组患者的临床总有效率为95.35%,较对照组患者的74.42%显著提高(P<0.05);治疗后观察组患者的TBIL、ALT、AST、BUN、Scr值均低于对照组患者,治疗后观察组患者IL-6、IL-8、IL-10、hs-CRP、TNF-α、IL-1β指标均低于对照组患者,差异均具有统计学意义(P<0.05)。结论谷氨酰胺联合早期肠内营养治疗急性重症胰腺炎可显著提高临床疗效,改善患者的肝功能指标,且细胞炎症因子水平显著降低,值得进一步在临床上推广应用。Objective To analyze the clinical efficacy of glutamine combined with early enteral nutrition in the treatment of severe acute pancreatitis and its influence on inflammatory factors. Methods Eightysix cases of patients with severe acute pancreatitis were selected and randomly divided into observation group and control group. Glutamine combined with early enteral nutrition therapy and simple early enteral nutrition therapy was given respectively. The clinical efficacy, liver and kidney function indexes, cytokine levels before and after treatment was compared between the two groups. Results The total effective rate of observation group was 95.35% which was significantly higher than the control group（P〈0.05）. After treatment, TBIL,ALT, AST, BUN and Scr values of the patients in observation group were lower than those in the control group, and values of IL-6, IL-8, IL-10, hs-CRP, TNF-α, and IL-1β were also lower than those in the control group, the differences were statistically significant（P〈0.05）. Conclusions Glutamine combined with early enteral nutrition in the treatment of severe acute pancreatitis could significantly improve the clinical efficacy,protect the liver function, and reduce the cytokine levels.
