摘  要:目的探讨日本血吸虫童虫排泄分泌抗原对血吸虫感染的诊断价值。方法收集日本血吸虫尾蚴,机械转化制备皮肤期童虫。收集童虫培养上清液,制备童虫排泄分泌抗原。用不同数量的尾蚴感染小鼠,收集不同感染度、感染时间的小鼠血清,采用基于血吸虫童虫排泄分泌抗原的酶联免疫吸附法检测抗童虫排泄分泌抗原抗体IgG水平的动态变化,观察其早期诊断价值。以同样方法检测健康人血清,血吸虫病患者及其他寄生虫病患者血清,观察该方法的临床诊断价值。结果小鼠感染血吸虫后1周血清中抗童虫排泄分泌抗原IgG即开始出现,并随着感染时间的延长抗体水平不断上升,5、10、15和20条尾蚴感染组小鼠在感染后1周抗体阳性率分别为40%、40%、80%和20%。用童虫排泄分泌抗原ELISA检测血吸虫病患者、健康人和其他寄生虫病患者血清均阳性。结论以日本血吸虫皮肤期童虫排泄分泌抗原ELISA检测相应IgG抗体对血吸虫初次感染小鼠具有早期诊断价值,可用于对血吸虫感染风险环境监测预警哨鼠的检测,但该方法尚不能用于人感染血吸虫的实验诊断。Objective To investigate the diagnostic value of excreted and secreted antigens of Schistosoma japonicum schistosomula. Methods Cercariae of S. japonicura were used to prepare skin-phase schistosomula via mechanical transformation. The supernatant of a schistosomulum culture was collected and concentrated to prepare the antigens ex- creted and secreted by schistosomula. Mice were infected with different number of cercariae, and serum samples were col- lected from mice before infection and at different time post-infection. Serum samples were also collected from patients with schistosomiasis, healthy individuals, and patients with other parasitic diseases. The sera of mice infected with differ ent number of cercariae at different times post-infection were examined for antigens excreted and secreted by schistosomu- la using an enzyme-linked immunosorbent assay. The dynamics of IgG antibodies against antigens excreted and secreted by schistosomula and the value of detecting this antibody for early diagnosis were observed. Serum samples from healthy individuals and patients with other parasitic diseases were also tested with ELISA to evaluate the clinical diagnostic value of antigens excreted and secreted by S. japonicum schistosomula. Results The level of IgG antibodies against antigens excreted and secreted by schistosomula began to rise one week post-infection and rose as the duration of infection in- creased. Sera from mice infected with 5, 10, 15, or 20 cercariae tested positive for IgG antibodies against antigens excre- ted and secreted by schistosomula at a rate of 40%, 40%, 80%, and 200%, respectively, indicating that detection of IgG antibodies against antigens excreted and secreted by S. japonicum schistosomula is of value in diagnosing a schistosome infection in sentinel mice. Serum samples from patients with schistosomiasis, healthy individuals, and patients with other parasitic diseases reacted strongly and non-specifically to the excreted and secreted antigens according to ELISA, which was unable to diff
