摘 要:目的探讨糖尿病前期患者血清维生素D水平的变化,为糖尿病前期危险因素研究及其防治提供新的线索。方法按2009年美国糖尿病学会(ADA)诊疗指南关于糖尿病前期诊断标准从门诊体检人群中筛选糖尿病前期人群325例,另筛选健康人203例作为对照组,检测人体参数和测量生化指标,用酶联免疫吸附试验法测定血清25-羟维生素D(25(OH)D)水平,应用Logistic回归分析糖尿病前期危险因素,按血清25(OH)D四分位值分层,分别计算糖尿病前期患病优势比OR值及95%置信区间。结果武汉地区27岁以上人群糖尿病前期患病率高达18.3%;糖尿病前期组血清25(OH)D水平显著低于对照组[(34.4±12.3)nmol/L VS(43.5±18.4)nmol/L,P<0.01];血清低水平25(OH)D组TG、HbAlc、HOMA-IR均高于高水平25(OH)D组(P均<0.05);Pearson相关分析显示25(OH)D浓度与BMI、收缩压、FPG、HbA1c、TG、TC、HOMA-IR呈负相关(P均<0.05);Logistic分析显示:25(OH)D与糖尿病前期呈负相关[β=-0.31,OR=1.42,P=0.03],低水平25(OH)D是糖尿病前期的危险因素,调整年龄、BMI、HOMA-IR等影响因素后,当血清25(OH)D水平达低于23.5nmol/L时,糖尿病前期患病风险显著提高[OR=1.46,95%CI(1.15-1.86),P<0.05]。结论血清25(OH)D水平与糖尿病前期发病呈明显负相关,低水平25-羟维生素D是糖尿病前期的独立危险因子。Objective To study the relationship between serum 25-hydroxyvitamin and prediabetes.Methods Based on the diagnostic criteria of diabetes meatus of the ADA(2009),325 patients were selected as prediabetes group,and203 healthy subjects were selected as control group.The biochemical parameters of human were measured and serum25-hydroxyvitamin D [25(OH)D]was measured by enzyme linked immunoassay.Logistic regression analysis was used to analyze the relationship between prediabetes and its related factors.The odds ratios and 95%confidence interval of diabetes were calculated respectively according to quartiles of serum 25(OH)D.Results The total prevalence of prediabetes among more than 27 years old in Wuhan city was 18.3%.The levels of serum 25(OH)D in the prediabetes group and control group were(34.4±12.3)nmol/L and(43.5±18.4)nmol/L,and there were significant differences among two groups(P〈0.01).Logistic analysis showed lower serum 25(OH)D level was a predictor of prediabetes[β=-0.31,OR=1.42,P=0.03].After adjusting for age,sex,BMI,blood pressure,TC,TG and HOMA-IR,the odds ratio of prediabetes was significantly Increased [OR=1.46,95%CI(1.15-1.86),P〈0.05]when serum 25(OH)D levels were below23.5nmol/L.Conclusion Lower serum 25(OH)D levels are associated with prediabetes,the decreased serum levels 25(OH)D increased onset risk of prediabetes.
