经皮输精管置管辅助经自然腔道精囊镜手术

摘　　要：目的 报告经皮输精管置管辅助经自然腔道精囊镜手术的初步经验.方法 自2015年7月至今共收治血精患者15例,平均病程23个月,其中精囊结石伴精囊扩张3例,精囊炎12例.术中首先采用24G套管针行阴囊段输精管穿刺,留置导管,亚甲蓝稀释液冲洗输精管,采用6Fr输尿管镜寻找前列腺小囊,其两侧可见亚甲蓝冲出确定射精管口,进入射精管口后进而进入精囊进行精囊镜操作.结果 15例手术14例成功,术后随访3～9个月,12例无血精再发.结论 经皮输精管置管辅助下可明确射精管口位置,有利于提高经自然腔道精囊镜手术的成功率.Objective To describe our experience of natural orifice seminal vesiculoscopy assisted with per-cutaneous vas deferens catheterization. Method From July 2015 to now , 15 hematospermia cases which included 3 Seminal vesicle stone cases and 12 seminal vesiculitis cases underwent the seminal vesiculoscopy assisted with percutaneous vas deferens catheterization. Firstly a 24G needle was used to puncture into and indwell in the scrotum segment of vas deferens . Then a 6Fr rigid ureteroscope was placed into the urethra and the orifice of e-jaculatory ducts besides the prostate lacunae were confirmed by the methylene blue injected from the 24G nee-dle. Lastly the ureteroscope went through the ejaculatory ducts into the seminal vesicle to perform the seminal ve-siculoscopy. Results A total of 14 of the 15 cases of endoscopic procedures were successfully performed. A total of 12 of the 15 cases had no hematospermia after a 3 -9 months follow - up. Conclusions Percutaneous vas def-erens catheterization can help to confirm the location of orifice of ejaculatory ducts and increase the rate of suc-cessful seminal vesiculoscopy.
