摘 要:目的探讨原发性高血压(EH)发病中H2S的变化及其与炎症因子IL-6的相关性。方法收集46例健康对照(正常对照组)和80例EH患者血浆,将EH患者分为药物控制组(n=47)和未使用药物控制组(n=33);根据舒张压值将EH患者分为A组(舒张压<90mm Hg,n=21)、B组(90~<100mm Hg,n=23)、C组(100~110mm Hg,n=18)及D组(>110mm Hg,n=18)。采用亚甲基蓝法测定血清H2S,双抗体夹心酶联免疫吸附试验(ELISA)检测血浆IL-6的浓度。结果 EH患者及A、B、C、D组血浆H2S水平[(8.50±0.59)、(8.94±0.62)、(8.64±0.52)、(8.20±0.41)、(8.23±0.46)μmol/L],均显著低于正常对照组[(9.29±0.91)μmol/L,P<0.01]。与EH患者未使用药物控制组比较,药物控制组血浆H2S水平显著增加[(8.37±0.55)μmol/L vs.(8.77±0.53)μmol/L],差异有统计学意义(P<0.01);与正常对照组比较,EH组患者血浆IL-6水平显著增加[(0.48±0.07)ng/L vs.(0.65±0.13)ng/L],差异有统计学意义(P<0.01)。EH患者使用药物控制组血浆IL-6水平较未使用药物控制组显著降低[(0.56±0.07)ng/L vs.(0.71±0.13)ng/L],差异有统计学意义(P<0.01)。EH患者血浆H2S浓度与脉压差呈正相关(r=0.42,P<0.01),与IL-6呈负相关(r=-0.38,P<0.01),与收缩压和舒张压无相关性(P>0.05)。结论 EH患者血浆H2S水平明显降低,且随着舒张压增加而降低,血浆H2S水平与IL-6显著负相关。EH患者血浆H2S可能参与了炎症诱导的EH发病过程。Objective To explore the correlation between the change of plasma hydrogen sulfide(H2S)level and interleukin-6(IL-6)in the pathogenesis essential hypertension(EH).Methods Plasma was collected from 46 normal controls(control group)and 80 patients with EH.The EH patients were divided into the medication control group(n=47)and non-medication control group(n=33).According to the diastolic pressure(DP)value,the EH patients were divided into the group A(DP90mm Hg,n=21),B(DP=90-100mm Hg,n=23),C(DP=100-110 mm Hg,n=18)and D(DP110mmHg,n=18).The methylene blue method was adopted to measure serum H2S level,and the plasma IL-6level was detected by using the double antibody sandwich ELISA.Results The plasma H2S level in the EH patients,group A,B,C and D were(8.50±0.59)μmol/L,(8.94±0.62)μmol/L,(8.64±0.52)μmol/L,(8.20±0.41)μmol/L and(8.23±0.46)μmol/L respectively,which were significant lower than(9.29±0.91μmol/L)in the control group(P〈0.01).Compared with the non-medication control group,the plasma H2S level in the medication control group was significantly increased,the difference was statistically significant[(8.37±0.55)μmol/L vs.(8.77±0.53)μmol/L,P〈0.01];compared with the control group,the plasma H2S level in the EH group was significantly increased,the difference was statistically significant[(8.37±0.55)μmol/Lvs.(8.77±0.53)μmol/L,P〈0.01].Compared with the control group,the plasma IL-6level in the EH group was significantly increased,the difference was statistically significant[(0.48±0.07)ng/L vs.(0.65±0.13)ng/L,P〈0.01];Compared with the non-medication control group,the plasma IL-6level in the medication control group was significantly decreased,the difference was statistically significant[(0.56±0.07)ng/Lvs.(0.71±0.13)ng/L,P〈0.01].Furthermore,the plasma H2S level had significantly positive correlation with the pulse pressure(R=0.42,P〈0.01),significantly negative correlation with the IL
