血培养三级报告的临床应用价值

摘　　要：目的探讨血培养三级报告对临床诊疗菌血症的指导和应用价值。方法对我院2015年1月至9月237份阳性血液标本进行三级报告,分析一级报告时间和准确性,比较直接药敏(二级报告)与仪器法(三级报告)药敏结果的二者符合率。结果 237份血培养阳性标本,共检出革兰阳性菌105例,革兰阴性菌126列,真菌6例。一级报告时间平均为29.5h,准确率为97.89%(232/237)。革兰阳性葡萄球菌直接药敏与仪器法药敏的平均符合率为93.45%,严重错误率为0.11%,重大错误率为1.48%,一般错误率为4.97%,革兰阴性菌直接药敏与仪器法药敏的平均符合率为90.87%,严重错误率为0.32%,重大错误率为2.38%,一般错误率为6.43%,总体符合率为91.92%。结论血培养阳性标本三级报告的建立,为临床诊疗提供了准确和快速的结果,具有较大的临床应用价值。Objective To explore the guidance and application values of blood culture three-level reports for clinical diagnosis and treatment of bacteremia. Methods We proceeded a three-level report on 237 positive blood samples from January to September 2015, analysed the first level report in regards to the time and accuracy, and compared the coincidence rate of the susceptibility results of direct susceptibility （second level report） and instrument method. Results There were 105 strains of Gram-positive bacteria, 126 strains of Gram-negative ones and 6 strains of fungi in the 237 positive blood samples. The first level report time was 29.5 hours on average and the accuracy was 97.89% （232/237） . For Gram-positive staphylococci, the coincidence rate of susceptibility between direct susceptibility and instrument method was 93.45 %, with the serious error of 0.11%, significant error of 1.48 %, and general errors of 4.97 %. While for Gram-negative bacilli, the coincidence rate was 90.87%, with serious error being 0. 32%, significant error being 2.38%, and general error being 6.43%. The overall coincidence rate was 91.92%. Conclusion The establishment of three-level reports on blood culture positive specimens provides accurate and quick results for clinical diagnosis and treatment, which has great clinical application value.
