中药复方联合西药治疗急性细菌性痢疾疗效及安全性Meta分析

摘　　要：目的:系统评价中药复方联合西药治疗急性细菌性痢疾的疗效及安全性。方法:计算机检索近30年公开发表的中药复方联合西药治疗急性细菌性痢疾的随机对照试验(RCT)研究文献,双人独立筛选判定后,采用Jadad量表进行质量评价,应用Rev Man 5.3软件对提取的数据资料进行Meta分析。结果:纳入文献12篇,共计病例1143例。中药复方联合西药能显著提高急性细菌性痢疾的治疗总有效率[Z=6.06,OR=6.87,95%CI(3.68,12.81),P<0.00001],并能有效缩短退热时间[Z=15.72,MD=-1.58,95%CI(-1.77,-1.38),P<0.00001]及止泻时间[Z=12.75,MD=-1.58,95%CI(-1.81,-1.33),P<0.00001];试验中未发现明显不良事件。结论:该研究当前证据表明,较单纯西医治疗,中药复方联合西药显著提高急性细菌性痢疾的临床疗效,且安全性较好。但该次研究纳入文献样本量较小,文献质量不高,证据强度有限,该结论需要更高质量的循证研究进一步支持。Objective：To evaluate the clinical efficacy and safety of intergrated Chinese medicine compound and Western medicine for acute bacillary dysentery. Methods：A systematic search on randomized controlled trial data over the past 30 years,was performed. Two reviewers extracted data independently,and used Reviewer Manager Software（Rev Man 5.3）for statistical analysis. Results：12 studies with 1143 patients were included in the systematic review. Clinical efficacy analysis showed that intergrated Chinese medicine compound and Western medicine can significantly improve the clinical tatal effective rate[Z=6.06,OR=6.87,95%CI（3.68,12.81）,P〈0.00001],shorten the defervescence time[Z=15.72,MD=-1.58,95%CI（-1.77,-1.38）,P〈0.00001],and shorten the time of diarrhoea[Z=12.75,MD=-1.58,95%CI（-1.81,-1.33）,P〈0.00001]. There was no remarkable adverse event. Conclusion：Based on the current evidence,intergrated Chinese medicine compound and Western medicine showed better efficacy than Western medicine for acute bacillary dysentery,and was safe. As the ample size is small and low-quality literatures,the strength of the evidence is limited. Higher quality evidence-based studies are needed.
