益气养阴祛瘀中药对干燥综合征患者血清TLR9表达影响

摘　　要：目的:观察益气养阴祛瘀中药对干燥综合征患者血清中Toll样受体9(TLR9)蛋白表达水平的影响。方法:选取40例原发性干燥综合征(p SS)患者,并随机分为西药组(口服羟氯喹治疗)和中药组(口服益气养阴祛瘀中药水煎剂治疗)各20例,连续治疗24周;选取10例健康体检者作为正常对照组。观察中药组和西药组患者治疗前后的临床症状(口干、眼干、关节痛)、血沉(ESR)、血清Ig G含量;通过酶联免疫吸附测定法(ELISA法)检测40例患者治疗前后及10例健康体检者的血清TLR9蛋白浓度。结果:两组患者经过24周治疗后的口干、眼干、关节痛症状均较治疗前有明显改善(P<0.05),中药组患者在治疗后,口干、眼干的症状改善优于西药组(P<0.05);两组患者经过24周治疗后ESR和血清Ig G均较治疗前下降(P<0.05);两组患者治疗前和治疗后的血清TLR9蛋白浓度均高于正常组(P<0.05);中药组患者治疗后的血清TLR9蛋白浓度较治疗前下降,其差异具有统计学意义(P<0.05);SS患者的血清TLR9蛋白浓度与口干VAS评分及眼干VAS评分存在相关性。结论:TLR9蛋白在SS疾病的发生发展中起着重要的作用;益气养阴祛瘀中药对SS具有较好的疗效,可明显改善患者口干、眼干症状,降低血沉和血清Ig G含量,其作用机制与抑制血清中TLR9蛋白的表达水平有关。Objective：To study the effect of Yiqi Yangyin Quyu Chinese herb on serum TLR9 protein express in patients with Sjogren＇s syndrome（SS）. Methods：40 patients with primary Sjogren＇s syndrome（p SS）were randomly and averagely divided into Western medicine group（oral hydroxychloroquine）and traditional Chinese medicine group（oral Chinese medicine）,treatment of 24 weeks,and observe the clinical symptoms（dry mouth,dry eye,joint pain）,erythrocyte sedimentation rate（ESR）,serum Ig G levels in before and after the treatment. Select 10 healthy physical examination as normal control group,then detect serum concentrations of TLR9 protein in patients with SS and healthy physical examination by enzyme-linked immunosorbent assay（ELISA）. Results：After 24 weeks of treatment,the clinical symptoms（dry mouth,dry eye,joint pain）of two group of patients significantly improved than before treatment;and Chinese medicine group of patients after treatment,the symptoms of dry mouth,dry eye improved better than Western medicine group;The ESR and serum Ig G of two group of patients were fall after 24 weeks of treatment,and no difference between the two group of patients at after 24 weeks of treatment;Serum TLR9 protein concentration in patients with SS was higher than the healthy physical examination;After treatment,serum TLR9 protein concentration of Chinese traditional medicine group in patients with SS was significantly decreased;The serum TLR9 protein concentration in SS patients had positive correlation with dry mouth and dry eyes. Conclusion：Yiqi Yangyin Quyu Chinese herb has good curative effect in SS,can significantly improve the dry mouth,dry eye symptoms,reduce blood sedimentation,the content of serum Ig G,its possible action mechanism is associated with inhibition of TLR9 protein expression level in serum.
