针刺治疗中风后尿潴留临床疗效及最大尿流率观察

摘　　要：目的:对比调补督任组与对照组针刺治疗中风后尿潴留的临床疗效,记录患者恢复自主排尿后最大尿流率,为临床治疗中风后尿潴留提供新的思路与方法,并为针刺治疗中风后尿潴留提供临床依据。方法:将80例中风后尿潴留患者随机分为两组,其中调补督任组50例、对照组30例。调补督任组选取百会、气海、关元、人中、内关为主穴,对照组选取中极、膀胱俞、秩边、阴陵泉、三阴交为主穴。14 d为1个疗程,各组均连续治疗3个疗程,治疗结束后评价疗效,并以每个疗程结束为时点,观察患者恢复自主排尿后最大尿流率。结果:调补督任组显著改善比例为46%,中度改善比例为40%,轻度改善为14%;对照组显著改善比例为43%,中度改善比例为40%,轻度改善比例为17%。结论:两组患者的临床症状均有不同程度的改善,调补督任组针刺效果明显好于对照组(P<0.05),提示"调补督任"针刺法能有效改善中风后尿潴留患者的临床症状,对改善患者尿流动力学指标有一定意义。Objective：To provide new clinical train of thought,method and clinical evidence for acupuncture in treating post-stroke uroschesis,by comparing the efficacy of acupuncture in the ＂regulating Du and Ren channel＂ group in treating post-stroke uroschesis to that in the control group,and recording the maximum urinary flow-rate after the automatic micturition recoverd. Methods：Eighty patients were randomly allocated to two groups,50 cases in the＂regulating Du and Ren channel＂group,30 cases in the control group. The＂regulating Du and Ren channel＂groups were treated with acupuncture at Baihui,Qihai,Guanyuan and Neiguan as main points,and the control group were treated with acupuncture at Zhongji,Pangguangshu,Yinlingquan and Sanyinjiao as main points. 14 days as 1 course of treatment,and each group had 3 course of treatment,after which the efficacy was evaluated and the automatic micturition of patients was observed after each course of treatment. Results：In the＂regulating Du and Ren channel＂group,the significant improvement percentage was 46%,the moderate improvement percentage was 40%,and the slight improvement percentage was 14%. In the control group,the significant improvement percentage was 43%,the moderate improvement percentage was 40%,and the slight improvement percentage was 17%. Conclusion：Clinical symptoms of patients in both groups improved with different degree. The efficacy of acupuncture in the ＂regulating Du and Ren channel＂group was significantly better than that in the control group,which showed that ＂regulating Du and Ren channel＂acupuncture can effectively improve the symptoms of post-stroke uroschesis,and had a certain significance on improving the urine dynamic index.
