温针灸联合耳针埋压治疗痰湿内阻型肥胖并发高脂血症疗效分析

摘　　要：目的:观察温针灸联合耳针埋压对女性痰湿内阻型肥胖并发高脂血症患者的干预效应,并分析影响疗效的因素。方法:选取来自2006年2月—2011年2月南京中医药大学国医堂门诊部单纯性肥胖病并发高脂血症痰湿内阻型患者113例,根据中医辨证论治基本理论,采用温针灸联合耳针施治,观察治疗3个月前后患者肥胖指标:体质量、肥胖度(A%)、体质指数(BMI)、体脂百分率(F%),脂质指标:总胆固醇(TC)、甘油三酯(TG)、低密度脂蛋白胆固醇(LDL-C)、高密度脂蛋白胆固醇(HDL-C)水平的变化和疗效。结果:温针灸联合耳针治疗后各肥胖指标及脂质指标均明显改善,与治疗前相比差异具有统计学意义,均P<0.01,总有效率为94.7%;疗效分析:不同性别、年龄段、病程与疗效比较差异具有统计学意义,均P<0.01;病因、诱因与疗效关系中,情志因素、嗜烟酒因素、过食少动引起者疗效最好,有效率分别为96.3%、94.4%、92.9%,更年期因素引起者疗效稍差,有效率为80%。结论:温针灸联合耳针埋压治疗痰湿内阻型肥胖并发高脂血症患者减肥、降脂疗效显著;年龄、性别、病程、情志、烟酒、过食少动等因素与疗效明显相关。Objective ： To observe the intervene effect of warming needle moxibustion with auricular acupuncture in treatment for obesity complicated with hyperlipidemia patients with phlegm - dampness stagnation type and analyze the factors which influence therapeutic effect. Methods： A total of 113 cases of obesity complicated with hyperlipidaemia on phlegm-dampness stagnation type, according to the Chinese medicine treatment, used warming needle moxibustion with auricular acupuncture. The changes in obesity index, total cholesterol （ TC ） , triglyceride （TG） , low density lipoprotein （ LDL - C ） and high density lipoprotein （ HDL - C ） in patient were observed before and 3 months after treatment. Re- sults ： Compared with the efficacy of obesity index and lipid level after treatment, patients all obtained significant effect in reducing weight and improving lipid （ all P 〈 0. 01 ）. The obesity index, TC, TG and LDL - C significantly decreased while HDL - C significantly increased. The total effective rate was 94.7%. There were significant negative correlations among the gender, course of disease, obesity degree and effect. The best therapeutic effect was caused by the tobacco and wine factors, mood factor and unheahby lifestyle. The less therapeutic effect was caused by climacteric factors. Conclusions： Warming needle moxibustion with auricular acupuncture treatment for obesity complicated with hyperlipidemia patients with phlegm - dampness stagnation type has a approving therapeutic effect. The patients＇ different obesity degree, the gender and course of disease are statistically significant.
