南昌市过敏性疾病患者血清过敏原特异性IgE分布的调查分析

摘　　要：目的了解南昌市过敏性疾病的种类及分布特点,为南昌市过敏性疾病防治提供参考。方法采用全自动蛋白印迹仪（免疫印迹法）检测3642例过敏性疾病患者20种过敏原[吸入性过敏原包括树组合（柳树/杨树/榆树）、普通豚草组合（西瓜/瓜/黄瓜/香蕉）、艾蒿组合（芹菜/胡萝卜/调料/扁豆/芥末/榛子）、尘螨组合（屋尘螨/粉尘螨）、屋尘、猫毛、狗上皮、蟑螂、霉菌组合（点青霉/分枝孢霉/烟曲霉/交链孢霉）、葎草;食物性过敏原包括鸡蛋白、牛奶、花生、黄豆、牛肉、羊肉、海鱼组合（鳕鱼/龙虾/扇贝）、淡水鱼组合（鲑鱼/鲈鱼/鲤鱼）、虾、蟹]血清特异性IgE（sIgE）抗体的水平。比较不同性别、不同年龄过敏原的差异。结果 3642例患者中,过敏原血清总IgE抗体阳性率为41.1%（1496/3642）。吸入性过敏原血清sIgE抗体阳性率为20.7%（754/3642）,食物性过敏原血清sIgE抗体阳性率为17.1%（624/3642）,吸入性过敏原血清sIgE抗体阳性率与和食物性过敏原比较差异有统计学意义（χ^2=151.126,P〈0.05）。男性的树组合、普通豚草组合、尘螨组合、屋尘、虾血清sIgE抗体阳性率与女性比较差异均有统计学意义（均P〈0.05）。0-10岁以尘螨组合、屋尘、艾蒿组合、花生、鸡蛋白等为主,11-20岁和〉20岁均以尘螨组合、屋尘、艾蒿组合、花生、蟹为主。尘螨组合、屋尘、鸡蛋白、花生和蟹的过敏原血清sIgE抗体阳性率在0-10岁、11-20岁和〉20岁间比较差异均有统计学意义（均P〈0.05）。结论南昌市人群过敏性疾病主要吸入性过敏原为艾蒿组合、尘螨组合和屋尘;主要食物性过敏原为蟹、花生和虾。Objective To understand the types and distribution characteristics of allergens in Nanchang,and to provide a basis for the prevention and treatment of allergic diseases.MethodsAutoblot system（immunoblotting）was used to detect the levels of serum IgE（sIgE）to 20 kinds of allergens in 3642 patients with allergic diseases.The inhaled allergens included tree combination（willow/poplar/elm）,common ragweed combination（watermelon/melon/cucumber/banana）,mugwort combination（celery/carrot/seasoning/lentils/mustard/hazelnut）,dust mite combination（house dust mite/dust mite）,house dust,cat hair,dog epithelium,cockroach,mould combination（penicillium notatum/mycobacterium/aspergillus fumigatus/alternaria）,and scandent hop.The food allergens included egg white,milk,peanut,soybean,beef,mutton,marine fish combination（cod/lobster/scallop）,freshwater fish combination（salmon/bass/carp）,shrimp,and crab.The allergens were compared between genders,as well as among different age groups.Results The total positive rate of sIgE antibodies was 41.1%（1496/3642）among the 3642 patients.The positive rate of sIgE antibodies to inhaled allergens（20.7%,754/3642）was significantly higher than that to food allergens（17.1%,624/3642）（χ^2=151.126,P〈0.05）.Furthermore,there were significant differences in the positive rates of sIgE antibodies to tree combination,common ragweed combination,dust mite combination,house dust and shrimp between genders（P〈0.05）.Dust mite combination,house dust,mugwort combination,peanut and egg white were the main allergens in 0-10-year-old group.Dust mite combination,house dust,mugwort combination,peanut and crab were the main allergens in both 11-20-year-old group and 〉20-year-old group.The differences in the positive rates of sIgE antibodies to dust mite combination,house dust,egg white,peanut and crab were significant among these three age groups（P〈0.05）.Conclusion Mugwort combination,dust mite combination and house dust are the main inhaled allergens and cra
